6.3.4 Registration with Care Quality Commission <strong>and</strong> periodic / special<br />

reviews<br />

<strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust is required to register with the Care Quality<br />

Commission <strong>and</strong> its current registration status is registered without conditions. <strong>Great</strong> <strong>Western</strong><br />

<strong>Hospital</strong>s NHS Foundation Trust has the following conditions on registration - none.<br />

<strong>The</strong> Care Quality Commission has not taken enforcement action against <strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s<br />

NHS Foundation Trust during 2011/12.<br />

<strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust has participated in special reviews or investigations<br />

by the Care Quality Commission relating to the following areas during 2011/12: -<br />

6. Chippenham 8 th November 2011 (Beech now Mulberry Ward)<br />

7. <strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong> 12 th December 2011 (<strong>The</strong>atres specialist review)<br />

8. <strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong> 12 th December 2011 (Privacy <strong>and</strong> Dignity follow up)<br />

9. <strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong> 8 th February 2012 (IR(ME)R Radiology specialist review-announced<br />

inspection)<br />

10. <strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong> 21 st March 2012 (Termination of Pregnancy)<br />

<strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust intends to take the following action to address the<br />

conclusions or requirements reported by the Care Quality Commission – comply with specific <strong>and</strong><br />

measurable actions plans developed after each inspection to address any issues raised in the<br />

inspection reports. In all cases internal assurance that all actions have been met will be sought <strong>and</strong><br />

internal CQC compliance monitoring processes are in place.<br />

<strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust has made the following progress by 31 March 2012<br />

in taking such action – none.<br />

6.3.5 Quality Data<br />

<strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust submitted records during 2011/12 to the Secondary<br />

Uses service for inclusion in the <strong>Hospital</strong> Episode Statistics which are included in the latest published<br />

data. <strong>The</strong> percentage of records in the published data:<br />

- which included the patient's valid NHS Number was: 99.5% for admitted patient care; 99.8% for<br />

outpatient care; <strong>and</strong> 92.2% for accident <strong>and</strong> emergency care. <strong>The</strong> lower performance in accident <strong>and</strong><br />

emergency care is attributed to the completeness of this data item at the minor injury units in<br />

Wiltshire <strong>and</strong> the Trust data quality group is working on improving this.<br />

- which included the patient's valid General Practitioner Registration Code was 99.6% for admitted<br />

patient care; 99.5% for outpatient care; <strong>and</strong> 100% for accident <strong>and</strong> emergency care.<br />

<strong>Great</strong> <strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust Information Governance Assessment <strong>Report</strong> overall<br />

score for 2011/12 was 77% <strong>and</strong> was graded satisfactory / green.<br />
