This has been achieved by:<br />

• Monthly reporting of percentage of patients having surgery within 36 hours<br />

• Monthly trend analysis to close any gaps identified<br />

• Monthly reporting of reasons for non-operation within 36 hours<br />

• Changes to processes to improve compliance<br />

• Prioritisation of operating slots for patients with hip fracture<br />

• Increased bank holiday/weekend trauma lists<br />

Priority 11: To ensure patients are assessed for the risk of developing<br />

Venousthromboembolisms <strong>and</strong> that these risks are managed appropriately<br />

Compliance with completing VTE risk assessment has been maintained at over 90%. This<br />

achievement includes data from Wiltshire since June 2011.<br />

This has been achieved with:<br />

• Continued education sessions at Trust Induction.<br />

• VTE update training now available on Training tracker <strong>and</strong> via a workbook.<br />

• VTE sessions provided specifically for Health Care assistants which was very successful <strong>and</strong><br />

enabled recruitment of additional VTE link HCA’s who have made significant improvements<br />

to the quality of VTE assessments in their areas. This is also being rolled out in Wiltshire to<br />

enable VTE link staff to have the same level of access.<br />

• Implementation of an audit trail through the nursing crescendo system <strong>and</strong> daily reports<br />

provided by Informatics to each ward area which allows them to easily identify any patients<br />

who have not been risk assessed.<br />

• Raising awareness with patients <strong>and</strong> relatives by means of information boards <strong>and</strong> displays<br />

during national thrombosis week, the winner of last year’s event is permanently displayed in<br />

Cherwell, pre-assessment unit.<br />

• A second patient information leaflet developed specifically for patients who are being<br />

discharged home with VTE prophylaxis which gives information for the patient <strong>and</strong> also the<br />

community health care provider.<br />

• We have also worked closely with Swindon PCT to establish VTE risk assessment in the<br />

community for patients who are discharged home with VTE prophylaxis. This will also enable<br />

patients who deteriorate at home to be assessed <strong>and</strong> for them to receive appropriate VTE<br />

prophylaxis if at risk.<br />

Administer appropriate VTE thromboprophylaxis<br />

Compliance with VTE prophylaxis has been maintained between 88%-100% for the last 12 months.<br />

Audits are proposed to evaluate the quality of the risk assessments <strong>and</strong> ensure appropriate<br />

thromboprophylaxis is prescribed <strong>and</strong> that patients who require extended prophylaxis in the<br />

community receive it.<br />
