Harm from falls WCHS: It was not possible to set a target for the reduction in numbers falls resulting<br />

in severe harm <strong>and</strong> death, as there was not a robust system in place to record the level of harm from<br />

reported falls prior to 2011/12. Since Wiltshire community services merged with GWH in June 2011,<br />

there have been two recorded severe harm incidents across the four community wards. <strong>The</strong>re has<br />

not been any death directly caused by falls in the same period of time.<br />

<strong>The</strong> previous Community Falls Policy has been updated; it now shares much of its contents with the<br />

revised acute falls policy, this is prior to the proposed full integration of both the community <strong>and</strong> acute<br />

polices later this year. Across the community hospital wards, the SAFE care bundle is being<br />

introduced.<br />

Priority 4: To reduce healthcare acquired pressure ulcers<br />

Pressure ulcers GWH: <strong>The</strong> combined target of both Category 3 <strong>and</strong> Category 4 hospital acquired<br />

pressure ulcers was 20 or less. <strong>The</strong> actual total number reported during 2011/12 was 5 (Grade 4 = 1;<br />

Grade 3 = 4).<br />

Comparing the total numbers for 2010/11 Category 4 <strong>and</strong> Category 3 hospital acquired pressure<br />

ulcers (13) with 2011/12 figures we have a 61.5% reduction on the previous year’s total number.<br />

Our greatest improvement is on the actual total numbers of pressure ulcers that have developed year<br />

on year. This year our strongest achievement has been in the reduction of Category 4s.<br />

Pressure ulcers are key quality care indicators within the Essence of Care patient-focused framework<br />

for clinical effectiveness <strong>and</strong> are included in the South West Key driver programme. Further<br />

information about pressure ulcers, including planned reductions, assessment tools, support, training<br />

<strong>and</strong> equipment can be found in the Trust’s Quality Account 2011/12.<br />
