6.8.4 Wiltshire Primary Care Trust<br />

NHS Wiltshire, as lead commissioner for Wilshire Community Health Services is pleased to assure<br />

the merged Wiltshire Community Health Services second annual Quality Account. <strong>The</strong> document is<br />

clearly presented in the format required by the Department of Health Toolkit <strong>and</strong> the information it<br />

contains accurately represents the Trust’s quality profile.<br />

This has been a year of transition for Wiltshire Community Health Services on 1 June 2011 <strong>Great</strong><br />

<strong>Western</strong> <strong>Hospital</strong>s NHS Foundation Trust acquired Wiltshire Community Health Services <strong>and</strong><br />

became a merged organisation. <strong>The</strong> Wiltshire Community Health Services have joined the<br />

Unscheduled <strong>and</strong> Community Care directorate, Children’s <strong>and</strong> the Bath Clinical Area Maternity<br />

Services have joined the Directorate of Women <strong>and</strong> Children's Services.<br />

Wiltshire Community Health services provides a range of general <strong>and</strong> specialist services, <strong>and</strong> it is<br />

right that these services should aspire to make year on year improvements in the st<strong>and</strong>ards of care<br />

they can achieve. We believe the specific priorities for 2012/13 which the trust has highlighted in the<br />

report are appropriate areas to target for continued improvement.<br />

NHS Wiltshire’s strategy for improving health <strong>and</strong> health care services in Wiltshire set out clear<br />

priorities for ensuring that wherever possible patients can be looked after in their own home <strong>and</strong> that<br />

they have access to services which offer excellence in terms of clinical outcomes <strong>and</strong> patient<br />

experience. <strong>The</strong> Quality Account acknowledges the level of the challenge posed in some 2011/12<br />

priorities, particularly in relation to patient falls. We support the Trust’s decision to continue to improve<br />

patient safety, ensure effective care <strong>and</strong> continue to improve the patient experience.<br />

We welcome the inclusion of success measures within the Quality Account, providing a gauge upon<br />

which service users, carers <strong>and</strong> commissioners can appraise the Trust’s achievements in the coming<br />

year. NHS Wiltshire looks forward to continuing to work with <strong>Great</strong> <strong>Western</strong> Foundation Trust as they<br />

fulfil their commitment to continuously improve the quality of care for our local health service users,<br />

their families <strong>and</strong> carers.<br />

6.8.5 Swindon Primary Care Trust<br />

NHS Swindon as lead commissioner has reviewed GWFT Quality <strong>Accounts</strong> report for 2011/12. <strong>The</strong><br />

Quality <strong>Accounts</strong> were reviewed by the Commissioning for Quality group which includes Clinical<br />

Commissioning group representation. NHS Swindon can confirm that, in their view, the Quality<br />

<strong>Accounts</strong> complies with the guidelines for application for the trust QA report.<br />

Commissioners monitor performance <strong>and</strong> the quality of services routinely each month with the Trust.<br />

Commissioners can confirm that, to the best of our knowledge, the Trust QA 2011/12 contains<br />

accurate information in relation to the services provided.<br />

<strong>The</strong> Trust has set their priorities by exploring multiple sources ranging from patient feedback to local<br />

intelligence collected via incident reporting <strong>and</strong> complaints, as well as consulting staff <strong>and</strong><br />

commissioners. <strong>The</strong> approach to setting priorities is commended by NHS Swindon <strong>and</strong> we are happy<br />

to endorse the targets that have been set. <strong>The</strong> monitoring of each priority is deemed to be set at<br />

appropriately timed intervals for each priority, allowing a timely response to address issues that may<br />

cause the target to be missed.<br />

It is good to see the improvements for quality of end of life care for patients <strong>and</strong> improve access to<br />

palliative care services. NHS Swindon would encourage the further integration <strong>and</strong> collaboration of<br />

health <strong>and</strong> social care community services as a driver to achieve this improvement.<br />
