Steering Group, provides a quarterly data quality <strong>and</strong> completeness report, including the results of<br />

data accuracy tests. <strong>The</strong> Data Quality Group also undertakes a Data Quality Work Programme,<br />

which includes data quality reporting, training <strong>and</strong> awareness, clinical coding, <strong>and</strong> policies <strong>and</strong><br />

procedures. <strong>The</strong>se corporate <strong>and</strong> operational arrangements ensure that information governance <strong>and</strong><br />

data quality are prioritised at all levels of the Trust.<br />

Each year the Trust completes a comprehensive self-assessment of its information governance<br />

arrangements by means of the NHS Information Governance Toolkit. <strong>The</strong> main Toolkit headings are:<br />

• Information Governance Management<br />

• Confidentiality <strong>and</strong> Data Protection Assurance<br />

• Information Security Assurance<br />

• Clinical Information Assurance – Health Records <strong>and</strong> Information Quality<br />

• Secondary Use Assurance<br />

• Corporate Information Assurance – Records Management <strong>and</strong> Freedom of Information.<br />

<strong>The</strong>se assessments <strong>and</strong> the information governance measures themselves are regularly validated<br />

through independent internal audit. <strong>The</strong> Trust’s Information Governance Assessment <strong>Report</strong> overall<br />

score for 2011/12 was 77% <strong>and</strong> was graded Green/Satisfactory, with a satisfactory rating in every<br />

heading of the Information Governance Toolkit.<br />

6.3.6 Explanatory Note for clinical Coding<br />

<strong>The</strong> Clinical Coding Audit carried out by the Audit Commission takes a sample of 100 patients from a<br />

selected specialty, in this years audit Trauma <strong>and</strong> Orthopaedics, as well as 100 patients r<strong>and</strong>omly<br />

selected across all specialties. <strong>The</strong> samples are therefore small <strong>and</strong> the results of the audit should<br />

not be extrapolated further than the actual sample audited.<br />
