_________________LEE UNIVERSITY FACULTY HANDBOOK & CONSTITUTION_________________plans will continue to be covered during the leave period as long as the employee portionof premiums continue to be paid. During the first sixteen weeks of leave, the universitywill continue to pay its portion of premiums. Beginning with the seventeenth week ofleave, the employee will be responsible for total premium costs. The universityrecognizes that individual capacities, roles, and circumstances differ. Therefore, facultymembers may vary in their use of parturition leave. The employee must give at leastthree months advance notice of intent to take such a leave (unless a medical emergencymakes the leave necessary) in order to be reinstated to the same or equivalent positionupon returning to work.Bereavement LeaveIn case of death in the immediate family, employees are granted, when necessary, upto three days leave with pay. Immediate family consists of spouse, parent, child, sibling,grandparent, grandchild, mother–in–law, father–in–law, daughter–in–law, son–in–law,spouse’s grandparent, or a relative living in the employee’s residence.FMLA LeaveUnder the Family and Medical Leave Act of 1993 (FMLA), qualified employees areentitled to up to 12 weeks of unpaid leave per year for the birth or adoption of a child, tocare for a spouse or an immediate family member with a serious health condition, orwhen unable to work because of a serious health condition. Employees must haveworked one year, and 1250 hours during the previous 12 months, in order to be eligiblefor Family and Medical Leave.An expansion of the FMLA became law in 2008 for employees who provide care towounded U.S. military personnel. Immediate family members (i.e. spouses, children orparents) of soldiers, reservists and members of the National Guard may qualify for up to26 weeks under the expanded Act.The employee must request a Family and Medical Leave through his/her supervisor.FMLA request forms are available in Human Resources. Employees participating in <strong>Lee</strong><strong>University</strong> insurance plans will continue to be covered during the leave period as long asthe employee continues payment for their portion of the insurance premiums. Theemployee will be reinstated to the same or an equivalent position upon return to workafter the leave period ends.Professional LeaveA faculty member may apply to the dean for a professional leave without pay. Thedean in consultation with the department chairperson will make a recommendation to theVice President for Academic Affairs, who will make a recommendation to the President.Other professional employees would apply to the President through their supervisor.Military Leave<strong>Lee</strong> <strong>University</strong> supports armed forces personnel by granting military leave when anemployee is called to active duty. When the necessity for military leave is known inadvance, the employee should provide his/her supervisor at least two weeks’ writtennotice. Otherwise, the employee should give notice as soon as possible. Military leave isunpaid. The employee will accrue seniority and benefits while on military leave. Theemployee should notify his/her supervisor at least two weeks prior to returning from6 - 2 Benefits of <strong>Faculty</strong> Service
