Grade Notification<br />
Grades are posted on MySCU approximately one week be<strong>for</strong>e the start of each new trimester. Unofficial<br />
transcripts may also be viewed on MySCU.<br />
Student ID Cards<br />
Students are issued identification cards from OneStop Enrollment Services during orientation. To receive a<br />
student ID card, students must provide their student ID number.<br />
Change of Personal In<strong>for</strong>mation<br />
Address/Telephone Number<br />
The student is responsible <strong>for</strong> notifying the Registrar’s Office in writing within 48 hours of any change in address<br />
<strong>and</strong>/or telephone number. The University is not responsible <strong>for</strong> any penalties assessed <strong>for</strong> failure to receive<br />
notices as a result of an incorrect address. Change of address <strong>and</strong> other <strong>for</strong>ms are available at:<br />

Name<br />
University policy requires an official certified/recorded document as proof of a name change. A Name Change<br />
Decree must have the signature <strong>and</strong> seal of the County Recorder’s Office. Contact the Registrar’s Office <strong>for</strong> more<br />
in<strong>for</strong>mation.<br />
Requests <strong>for</strong> Transcripts<br />
To request transcripts, submit a completed Transcript Request Form (available at<br />
www.scuhs.edu/resources/registrar) to the Registrar’s Office via mail, fax, or email.<br />

Cost of transcripts is $10.00 per transcript, per program. Additional fees <strong>for</strong> services may apply. Fees may be<br />
paid by check, cash, money order, or credit card.<br />
Regular service (pick up or first-class USPS mail processed within five business days of receipt) is free. No<br />
tracking in<strong>for</strong>mation or delivery receipt is available <strong>for</strong> this service.<br />
Rush service (request submitted be<strong>for</strong>e 5:00pm) fee is $15.00. Requests are processed the following business<br />
day <strong>and</strong> transcripts are ready <strong>for</strong> pick up or are mailed first-class USPS mail. No tracking in<strong>for</strong>mation or delivery<br />
receipt is available <strong>for</strong> this service.<br />
Expedited processing <strong>and</strong> delivery (request submitted be<strong>for</strong>e 5:00pm) fee is $15.00 plus $22.95 <strong>for</strong> USPS Priority<br />
Mail Express or an applicable FedEx charge. Tracking in<strong>for</strong>mation is provided.<br />
