Ordinary Meeting <strong>of</strong> Council - 18 July 2006 4 / 2 Item 4 S02957 29 June 2006 PURPOSE OF REPORT For Council <strong>to</strong> consider the long term viability <strong>of</strong> maintaining a childhood immunisation clinic on <strong>be</strong>half <strong>of</strong> NSW Health. BACKGROUND Ku-ring-gai Council has mainitained childhood immunisation clinics on a monthly basis over many years, operating both a morning and evening clinic on the third Wednesday <strong>of</strong> each month. The commencement date <strong>of</strong> these clinincs is not known, however, a search <strong>of</strong> Council records reveals personnel immunisation records extending back <strong>to</strong> the 1950s. Additional <strong>to</strong> the monthly clinics , special clinics have also <strong>be</strong>en conducted from time <strong>to</strong> time in conjunction with programmes developed by the Department <strong>of</strong> Health such as the national Measles and Mumps campaign that targetted all children , including school age children in a catch up programme. Service provision Initially, Council provided immunisation against Poliomyelitis , Dipteheria, Tetnus and Whooping Cough. Today, in line with medical advances, Council <strong>of</strong>fers immunisation against a wide range <strong>of</strong> diseases. The current immunisation service provided is as follows: Age Disease immunised against 2 months Poliomyelitis, Diptheria, Tetanus, Pertusis, Hepatitis B, Hib and Pneumoccoccal 4months Poliomyelitis, Diptheria, Tetanus, Pertusis, Hepatitis B, Hib and Pneumoccoccal 6 months Poliomyelitis, Diptheria, Tetanus, Pertusis, Hepatitis B, Hib and Pneumoccoccal 12 months Measles, Mumps, Ru<strong>be</strong>lla, Hepatitis B, HIB, Meningococcal C 18 months Chickenpox 4 years Poliomyelitis, Diptheria, Tetanus, Pertusis, Measles, Mumps , Ru<strong>be</strong>lla Clinic attendance rates Immunisation clinics have generally <strong>be</strong>en satisfac<strong>to</strong>rily attended in recent years. However, in general terms there has <strong>be</strong>en a shift. In years past, it was normal for up <strong>to</strong> 120 children <strong>to</strong> attend, however in recent years monthly attendance is averaging at around 50 per month. Some <strong>of</strong> this trend is attributed <strong>to</strong> demographics, but some must also <strong>be</strong> attributed <strong>to</strong> the preferred choice <strong>of</strong> parents <strong>to</strong> take their children <strong>to</strong> a private doc<strong>to</strong>r <strong>of</strong> their own choosing. Latest census data reveals 5,387 Children under 5 years <strong>of</strong> age live in Ku-ring-gai. 392 are clients <strong>of</strong> <strong>council</strong>'s immunisation service (7%) It is worthy <strong>to</strong> note that the mid morning clinic is the most popular, with 615 appointments as opposed <strong>to</strong> 123 appointments for the evening clinic in 2005 N:\060718-OMC-SR-03451-CHILDHOOD IMMUNISATION CL.doc/murphy /2

This low rate in Ku-ring-gai is attributed <strong>to</strong> poor reporting by GPs <strong>to</strong> the national immunisation register rather than non-immunisation. (The greater use <strong>of</strong> private GPs <strong>to</strong> provide immunisation for Ku-ring-gai children further enforces this.) Staffing and costs The clinics conducted by Council in conjunction with NSW Health, up unitl May 2006 ,were staffed by one medical practitioner, three immunisation nurses and up <strong>to</strong> three administration <strong>of</strong>ficers <strong>of</strong> Council. The provision <strong>of</strong> the medical practitioner ceased on the 16 May 2006, with her resignation. NSW Health has advised that she will not <strong>be</strong> replaced and that future clinincs will <strong>be</strong> staffed by immunisation nurses only. The immunisation nurses are employed by Council on a casual basis, with their costs <strong>be</strong>ing reimbursed by NSW Health on an annual basis. Administration staffing costs are borne by Council. Currently, the monthly clinic amounts <strong>to</strong> a monthly cost <strong>of</strong> $900 <strong>to</strong> Council. This cost includes staffing ($759) <strong>of</strong> clinic morning and evening, prior administration in terms <strong>of</strong> preparation in sending invitations and ensuring adequate medical supplies ($60), record keeping, and clininc set up ( $80). Vaccine is provided by NSW Health free <strong>of</strong> charge, however, any vaccine that is spoilt must <strong>be</strong> paid for by Council. The cost <strong>of</strong> providing the immunisation service is approximately $14 - $20 per child attending, by way <strong>of</strong> in-kind administrative support and the supply <strong>of</strong> ancillary pharmecuticals such as needles, skin wipes etc. Location As previously stated, clinics are <strong>held</strong> on a monthly basis. Clinics are conducted within the Council admininstration building on Level 3, (in the foyer area immediately outside the Council cham<strong>be</strong>r). The location <strong>of</strong> the clinics is not ideal, in so far as adequate provision cannot <strong>be</strong> made for either attending medical staff (space, running hot and cold water, examination tables) or the clients (privacy, quiet room, ramp access, threat <strong>of</strong> staircase).
