) Entitlements to long service leave are o<strong>the</strong>rwise as determined by <strong>the</strong> LongService Leave Act 1976.c) On resignation after completing five years of service, an employee shall beentitled to a pro rata long service leave payment in lieu of any outstandingLong Service Leave. Any such payment shall be calculated on <strong>the</strong> basis of oneweek for each week of completed year of service less <strong>the</strong> amount of LongService Leave already taken, provided that, no employee will be required torefund to <strong>the</strong> employer any difference in entitlement between leave taken andpayment in lieu of leave.d) An application for Long Service Leave must be made to <strong>the</strong> ManagementCommittee Meeting and approved by <strong>the</strong> Management Committee. Threemonths notice must be given before leave is taken.The above clauses are from <strong>the</strong> TU conditions of employment.
