ABSTRACT<br />

The purpose of this study is to analyze the literature of the <strong>paradental</strong> cyst,<br />

presenting a detailed review in relation to the etiology, clinic, radiographic<br />

characteristic, differential diagnostic, histopathological features, treatment and<br />

prognostic. The study defines the <strong>paradental</strong> cyst as odontogenic inflammatory origin<br />

mainly occurring in the vestibular region and or distal, rarely in the mesial region of<br />

the lower vital molar. The lesion can be extended for periapical region, provided that<br />

the bigger frequency is the cases that appear in the third lower molar partially<br />

outbroken and impelled, with the history of recurrent pericoronaritis and absence of<br />

generalized periodontal illness. The lesion can be presented as a subclinical infection<br />

only confirmed histological, but the history of pericoronaritis and or presence of<br />

signals and inflammatory symptoms are common. The radiographic characteristics<br />

show small dependent variations of the localization of the lesion in the third lower<br />

molar, and when associated with the first lower molar is common to find ossifying<br />

periostitis. The mandibular infected buccal cyst can be considered a variant but not a<br />

separate pathological entity of the <strong>paradental</strong>. The study suggests that some<br />

<strong>paradental</strong> cysts have been characterized as other denominations or even with<br />

incorrect diagnosis. The epithelial origin of this cyst isn’t completely clarified yet,<br />

moreover it foments other quarrels. Several approaches are being verified regarding<br />

to the treatment and the maintenance of the involved tooth and others with the<br />

exodontics of the involved dental element presenting changeable justifications<br />

related to the localization and the distinguishing diagnosis. The study concludes that<br />

the acquirements of the physiopathology differential diagnosis of the lesion,<br />

independent of its nomenclature, is primordial, through clinical and radiographic<br />

characteristics to obtain a correct diagnosis and consequently the most adequate<br />

and conservative treatment possible.<br />
