TBEBENEFITS SECTION<br />

This section <strong>of</strong> the manual is designed 10 acquaint employees with some <strong>of</strong> Ihe<br />

significanl features <strong>of</strong> the employer's benefit programs, including programs for<br />

time away from work. However, it is important to remember that more detailed<br />

information is set forth in the <strong>of</strong>ficial plan documents and insurance policies that<br />

govern the individual benefit pl;;ms. Accordingly, jf there is any real or apparent<br />

conflict between the brief summaries contained in this manual and the terms,<br />

conditions or limitations <strong>of</strong> the <strong>of</strong>ficial plan documents, the provisions <strong>of</strong> the<br />

<strong>of</strong>ficial plan documents will control. Employees who wish to inspect those<br />

documents can make an appOintment with the Human Resources Representative<br />

for that purpose.<br />

In addition, while it is the employer's present intention to continue these benefits,<br />

the employer reserves the right, whether in an individual case or more generally,<br />

to modify, curtail, reduce or eliminate any benefit, in whole or in part, either with<br />

or without notice.<br />

MEDICAL INSURANCE<br />

Eligible employees may participate in the company's medical insurance plan.<br />

Employees, whose regularly scheduled hours total 35 or more per week, are<br />

eligible for insurance on the first day <strong>of</strong> the month following completion <strong>of</strong> the<br />

three month introductbry Reriod. On-call and training employees are not eligible<br />

to participate in this plan. The company pays a percentage <strong>of</strong> the contracts<br />

according to a scale. Complete information regarding the scale <strong>of</strong> copayment, as<br />

well as a booklet containing the details <strong>of</strong> and eligibility requirements for the<br />

medical insurance plan may be obtained from the Human Resources<br />

Representative .<br />

Upon termination, the employee may be entitled to continuation or conversion <strong>of</strong><br />

the group medical insurance plan in accordance with the terms <strong>of</strong> the policy<br />

. and/or applicable state and federal law. For more information contact the Human<br />

Resources Representative.<br />

An employee cannot make changes to the medical insurance coverage until the<br />

nexf enrollment date, unless family status changes. Such changes include<br />

marriage or divorce, creation or dissolution <strong>of</strong> a domestic partnership, death <strong>of</strong> a<br />

spouse/domestic partner or child, birth or adoption <strong>of</strong> a child, or termination <strong>of</strong><br />

employment <strong>of</strong> an employee's spouse/domestic parlner. Abilily 10 make changes<br />

is governed by slate and federal law, and by Ihe applicable policy documents.<br />
