STATE DISABILITY INSURANCE<br />

To protect employees who mis$ work due to a non-work-related accident or<br />

illness, the law requires that a sma ll percentage <strong>of</strong> each employee's wages, up to<br />

the prevailing maximum, be deducted each pay period for disability insurance.<br />

The benefits are calculated as a percentage <strong>of</strong> salary, up to a weekly maximum<br />

as specified by law.<br />

The employee Is responsible for fi ling his or her claim and other forms promptly<br />

and accurately with the Employment Developn1enl Department. A claim form<br />

may be obtained from any <strong>of</strong>fice <strong>of</strong> the Employment Development Department by<br />

telephone, letter, or in person.<br />

SOCIAL SECURITY<br />

Employees are covered under the provisions <strong>of</strong> th e federal social security law<br />

(FICA). Social security benefits are <strong>of</strong>ten a significant step to provide an<br />

employee and his/her family a retirement income. The amount <strong>of</strong> the deduction<br />

from wages for social security taxes is matched by the compariy. The total<br />

contribution by the employee and the company is credited toward the employee's<br />

social security benefits, which may be available at the time the employee is<br />

eligible to retire, or in cases <strong>of</strong> disability and/or survivor benefits as defined by<br />

law.<br />

WORKERS' COMPENSATION<br />

On-the-jop inJuries are covered by tile company's workers' compensation<br />

Insurance policy. This insurance is provided at no cost to the employee. If an<br />

employee is- injured on the job, no matter how slightly. s/he must report the<br />

incident immediately to the supervisor. An injured employee will be taken to a<br />

nearby urgent care center or hospital if the injury cannot be treated adequately<br />

on the premises. Consistent with applicable state law, failure to report an injury<br />

wrthin a reasonable period <strong>of</strong> time could jeopardize the claim.<br />

The company asks for employee assistance in alerting management to any<br />

condition that could lead to or contribute to ah employee accident.<br />
