Note: A calendar year is January 1 to December 31.<br />
TERMINATION OF BENEFITS<br />
Coverage for you and your dependents will cease on the earliest <strong>of</strong>:<br />
(a)<br />
(b)<br />
the date your employment terminates;<br />
the date on which you early retire under the criteria <strong>of</strong> your pension<br />
plan.<br />
Benefits will be available under the Retiree group to any Employee meeting the<br />
following criteria;<br />
A former Employee who:<br />
(a)<br />
(b)<br />
(c)<br />
retired from the Employer under the <strong>City</strong> (HSR) Pension Plan<br />
factor; or,<br />
retired from the Employer early on the <strong>City</strong> (HSR) Pension Plan, is<br />
between the ages <strong>of</strong> 55 and 65, and, at the date <strong>of</strong> retirement had<br />
twenty (20) continuous years <strong>of</strong> employment with the Employer; or,<br />
was terminated for non-disciplinary reasons, while in receipt <strong>of</strong> LTD<br />
benefits:<br />
is eligible for the following benefits,<br />
(i) Extended Medical Plan<br />
(ii) Dental Plan<br />
(iii) Vision Plan<br />
subject to the conditions that,<br />
(i)<br />
(ii)<br />
these benefits will only be provided if similar coverage is not available to<br />
the former Employee from another source; and,<br />
these benefits will terminate on the last day <strong>of</strong> the month in which the<br />
former Employee attains the age <strong>of</strong> 65 years. The benefits continue for<br />
the spouse and dependents until the spouse attains the age <strong>of</strong> 65; and,<br />
