(4) Payments will be made for a maximum <strong>of</strong> twenty-six (26) weeks<br />

during any one continuous period <strong>of</strong> disability. Successive<br />

absences due to the same or a related cause will be considered<br />

one continuous period <strong>of</strong> disability unless separated by return to<br />

active employment for a period <strong>of</strong> three (3) months. A disability due<br />

to a different cause will be considered a new period after a return<br />

to active employment for one month.<br />

(5) No benefits will be payable during a period <strong>of</strong> pregnancy leave <strong>of</strong><br />

absence to which an Employee is entitled under the Employment<br />

Standards Act, or during any such longer period <strong>of</strong> pregnancy leave<br />

for which the Employee has applied and been approved by the<br />

Employer.<br />

An Employee shall be provided up to two (2) one-half (1/2) day<br />

absences for doctor appointments in any calendar year. Each <strong>of</strong><br />

these one-half (1/2) day absences shall not constitute an occasion<br />

for the purpose <strong>of</strong> this plan.<br />

Short term disability payments will be <strong>of</strong>fset by any disability<br />

benefits payable to the Employee from the Canada Pension Plan.<br />

REGULATIONS<br />

An Employee shall on the first (1st) day <strong>of</strong> illness, report or<br />

cause to report such illness to the Employee's immediate<br />

supervisor.<br />

An Employee who fails to report on the first (1st) day absent<br />

from work due to illnessshall be considered as being absent<br />

without leave and is subject to having their name removed<br />

from the payroll on instructions from the Director <strong>of</strong><br />

Employee and Labour Relations.<br />

Upon receiving notice <strong>of</strong> Employee's illness, the Supervisor<br />

shall on the same day report such illness on the "absence<br />

report forms".<br />

An Employee whose illness extends to the sixth (6th) working<br />

day shall, on or before the sixth (6th) working day, file a Sick<br />

Claim Form, attached as Appendix "B" with the Return to<br />

Work Services. The cost for completion <strong>of</strong> the initial form<br />

shall be the responsibility <strong>of</strong> the Employee, provided the<br />

initial form was fully completed, costs associated with<br />

subsequent requests for additional medical form or requests<br />
