Liječnik mora bolesniku izložiti sve prednosti i neželjene pojave svake
terapijske opcije. Informacija nije znanje. Znanje bez iskustva i kliničke
prosudbe nije mudrost. Liječnik mora ponekad amortizirati nerealistične
zahtjeve i mora znati odvagnuti između bolesnikovih želja, nadanja i pravila
medicinske struke.
ETHICAL DILEMMAS IN USE OF BIOLOGICS IN
DERMATOLOGY
There are four registered biological drugs that can be used in the treatment
of psoriasis: adalimumab, infliximab, ustekinumab and etanrecept.
Use of biologics should be based on the ethical principles of best patient
benefit, avoiding harm, patient autonomy in choosing their treatment and
fairness in relation to the limited health resources.
Fifty-five years old patient was admitted with psoriasis that lasted five
years affecting 20% of the body surface with no sign of psoriatic arthritis.
The patient had psychological trauma in communicating with people. He
had heard of biological therapy and requested the introduction of biological
therapies. Patient’s physician must explain all the benefits and side effects
of each treatment option. Information is not knowledge. Knowledge
without experience and clinical judgment is not wisdom. The physician
must amortize sometimes unrealistic demands and needs of patients. It is
important to decide between a patient’s wishes and hopes, and the guidelines
of the medical profession.
