Personnel: Inattention to fueling activities and failure to report the<br />

second spill. -<br />

Material: Failure of the fueling nozzle automatic shut off function.<br />

5. Description of Event/Occurrence:<br />

Employee was dispensing fuel from tank truck, 68C 4365, (equipped with three<br />

separate storage tanks and dispensing systems) into underground fuel tanks at the<br />

100-N fueling station. He placed the fuel dispensing nozzle into the 4" fill<br />

spout of the service station tank, set the automatic shut-off and returned to his<br />

truck to complete related paperwork while waiting for the tank to fill. The<br />

nozzle did not shut off and the tank overflowed. The employee did not observe<br />

the spillage until a passerby walked up to the truck to inform him of the<br />

overflowing condition. The fuel truck driver immediately turned the nozzle off<br />

manually. The passerby noted the fuel truck driver was in the truck attending<br />

paperwork and .wearing radio headphones which may have'contributed to his<br />

inattention to fueling activities. The spill was reported to N-Reactor<br />

Operations Management and the Supervisor, Special Delivery Services. Initial<br />

assessment of the spill area was done by N-Reactor Operations Management<br />

personnel. Environmental Assurance was advised.<br />

The employee then proceeded to manually fill the second underground tank with the<br />

remaining fuel in the first tank of the truck. When that truck tank was drained,<br />

he stored the hose and nozzle and proceeded to continue filling the second<br />

service station tank from the second truck tank with a different nozzle. He set<br />

the new nozzle on automatic and stepped back to remain clear of the fumes.<br />
