18. IMMEDIATE ACTIONS TAKEN AND RESULTS<br />

Sampling was completed, then additional sampling was<br />

conducted to determine how deep the contamination<br />

penetrated, this sampling revealed that the contamination<br />

had not penetrated below four inches from the bottom of the<br />

tank. <strong>Hanford</strong> Surplus Facility Program Office (Mike<br />

Mihalic) was notified of the leak at 1400 hours December 18,<br />

1990. The Programs Office notified Environmental<br />

Protection. As this project is being performed under<br />

approved decommissioning work procedures, and it had been<br />

anticipated that the soil contamination could/would be<br />

found, this Occurrence Report is submitted for notification<br />

purposes. A complete detailed report will be prepared and<br />

submitted upon completion of the sample analysis. By copy<br />

of the unusual occurrence report, Environmental Engineering<br />

is requested to perform the necussary evaluation,<br />

notification, and remedial action as required by 40 CFR<br />

280/281.<br />

22. DESCRIPTION OF CAUSE<br />

From reconstructing the most likely cause of the<br />

contaminated soil found during the removal of tank IOON-27,<br />

it was observed that the tank condition did not indicate the<br />

tank to be a leaker. There has been one reportable spill at<br />

this location in 1988. It is assumed there have been other<br />

unreported spills at this tank fill location. The gravel<br />

and porous backfill surrounding the tanks creates a natural<br />

pathway for surface spills, to the location where the<br />

contaminated soil was found.<br />
