Medical Center. Dr. Reed is also an affiliate investigator<br />

at the Center for <strong>Health</strong> Studies <strong>and</strong> Fred<br />

Hutchinson Cancer Research Center, <strong>and</strong> an associate<br />

pr<strong>of</strong>essor <strong>of</strong> obstetrics <strong>and</strong> gynecology, <strong>and</strong> an<br />

adjunct pr<strong>of</strong>essor in epidemiology at the University<br />

<strong>of</strong> Washington School <strong>of</strong> <strong>Medicine</strong>. Dr. Reed’s areas<br />

<strong>of</strong> clinical <strong>and</strong> research interest are gynecologic<br />

issues for women with genetic diseases, cross-cultural<br />

medicine, <strong>and</strong> management <strong>of</strong> menopause.<br />

Dr. Reed received her master’s degree (M.S.) in<br />

human genetics from Sarah Lawrence College in<br />

Bronxville, New York, her medical degree (M.D.)<br />

from Stanford University Medical Center in Stanford,<br />

California, <strong>and</strong> her Master <strong>of</strong> Public <strong>Health</strong><br />

(M.P.H.) degree from the University <strong>of</strong> Washington<br />

School <strong>of</strong> Public <strong>Health</strong>. Dr. Reed is a licensed<br />

genetic counselor <strong>and</strong> board-certified in obstetrics<br />

<strong>and</strong> gynecology.<br />

Jerry Richard Shields, M.D., is an ophthalmologist<br />

in private practice in Tacoma, Washington.<br />

He sees general ophthalmology patients <strong>and</strong> performs<br />

laser correction <strong>and</strong> cataract surgery. Dr.<br />

Shields is board-certified in ophthalmology <strong>and</strong><br />

VISIX-certified.<br />

Dr. Shields received his bachelor <strong>of</strong> science<br />

(B.S.) degree in biology cum laude from Wake Forest<br />

University in Winston-Salem, North Carolina,<br />

<strong>and</strong> his medical degree (M.D.) from the Medical<br />

College <strong>of</strong> Georgia in Augusta, Georgia. He completed<br />

his internship in internal medicine <strong>and</strong> his<br />

residency in ophthalmology at the University <strong>of</strong><br />

Washington in Seattle, Washington.<br />

Christina M. Surawicz, M.D., is gastroenterology<br />

section chief <strong>of</strong> at Harborview Medical Center<br />

in Seattle, Washington. She is also pr<strong>of</strong>essor <strong>of</strong><br />

medicine <strong>and</strong> assistant dean for faculty development<br />

at the University <strong>of</strong> Washington School <strong>of</strong><br />

<strong>Medicine</strong> in Seattle. Dr. Surawicz is widely published<br />

in peer-reviewed journals <strong>and</strong> has<br />

authored, edited, <strong>and</strong> contributed to dozens <strong>of</strong><br />

books.<br />

Dr. Surawicz received her undergraduate<br />

degree, a bachelor <strong>of</strong> arts (B.A.) in biology, from<br />

Barnard College in New York City <strong>and</strong> her medical<br />

degrees (M.D.), awarded with honors, from the<br />

University <strong>of</strong> Kentucky College <strong>of</strong> <strong>Medicine</strong> in<br />

Lexington, Kentucky. She completed her medical<br />

internship, medical residency, <strong>and</strong> gastroenterology<br />

fellowship at the University <strong>of</strong> Washington in<br />

Seattle. Dr. Surawicz is board-certified in internal<br />

medicine <strong>and</strong> gastroenterology.<br />

Denise L. Wych, R.N. C.M., is a cardiac nurse<br />

case manager at Palmetto <strong>Health</strong>-Richl<strong>and</strong> Heart<br />

Hospital in Columbia, South Carolina, where she<br />

integrates clinical <strong>and</strong> medical appropriateness criteria<br />

in discharge planning <strong>and</strong> resource utilization<br />

within the framework <strong>of</strong> an interdisciplinary<br />

health-care team. She has worked in medical-surgical,<br />

intensive care unit, newborn, school nursing,<br />

<strong>and</strong> home health.<br />

Ms. Wych received her diploma in nursing<br />

from Providence School <strong>of</strong> Nursing in S<strong>and</strong>usky,<br />

Ohio. She served 10 years as an <strong>of</strong>ficer in the US<br />

Navy Nurse Corps, during which she was twice<br />

awarded the Navy Achievement Medal.
