Aan patiënten met HCC en Child C worden enkel supportieve zorgen voorgesteld. Het risico van
morbiditeit en mortaliteit door cirrose doorkruist een mogelijk therapeutisch effect van een
oncologische behandeling .
Bij pat. met Child A/B cirrose moeten de specifieke behandelingsmogelijkheden in het MOC
worden besproken.
C/ BCLC –schaal
° Barcelona Clinic Liver Caner
° omvat ook informatie over tumor stadium, leverfunctie en fysieke status. Op die manier heeft
deze classificatie meer predictieve waarde bij de therapeutische planning dan de TNM-staging.
°BCLC A : deze pat. komen in aanmerking voor heelkundige/ablatieve therapie en transplantatie.
BCLC B : patiënten komen in aanmerking voor regionale therapie zoals TACE.
BCLC C: patiënten komen in aanmerking voor systemische behandeling of clinical trials.
D/ MELD-score
MELD is a prospectively developed and validated chronic liver disease severity scoring system that
uses a patient's laboratory values for serum bilirubin, serum creatinine, and the INR to predict
survival. The MELD score, as currently used by UNOS in prioritizing allocation of organs for liver
transplantation, is calculated according to the following formula:MELD = 3.8[Ln serum bilirubin
(mg/dL)] + 11.2[Ln INR] + 9.6[Ln serum creatinine (mg/dL)] + 6.4
Laboratory values of INR, total bilirubin and serum creatinine that are 4.0 mg/dL are capped at 4.0 mg/dL, and patients on dialysis
receive an assigned serum creatinine value of 4.0 mg/dL.
