8 Gastro-intestinale neuro-endocriene tumoren (GEP-NET) 8.1 Inleiding Jaarlijks worden in België 250 à 500 (5/100.000) nieuwe GastroEnteroPancreatische (GEP-) neuroendocrine tumoren (NET) gerapporteerd. Deze tumoren gaan uit van het maagslijmvlies, het pancreas, de dunne of dikke darm en de ampulla van Vater. De toename van incidentie in de laatste decaden houdt ongetwijfeld verband met betere diagnostische middelen ( endoscopie/CT/NMR) en betere registratie. Op 4/10/2011 keurde de ethische commissie van <strong>Sint</strong>- <strong>Lucas</strong> goed dat patiënten met diagnose van een GEP-NET – na informed consent van patiënt en anoniem- zouden opgenomen worden in een nationale databank (DNET) welke gecoördineerd wordt via de BGDO. De GEP-NET treden vnl. op vanaf 50jaar. Carcinoïd van de appendix en in de context van het MEN1-syndroom treden op jongere leeftijd op . a) Human GEP-NETs by site of origin and by symptom carcinoids (about two thirds of GEP-NETs) o with carcinoid syndrome (about 10 percent of carcinoids) o without carcinoid syndrome (about 90 percent of carcinoids) PETs (about one third of GEP-NETs) o nonfunctioning (15 to 30 percent of PETs) o functioning (70 to 85 percent of PETs) gastrinoma, producing excessive gastrin and causing Zollinger-Ellison Syndrome (ZES) insulinoma, producing excessive insulin glucagonoma, producing excessive glucagon vasoactive intestinal peptideoma (VIPoma), producing excessive vasoactive intestinal peptide (VIP) PPoma, producing excessive pancreatic polypeptide (often classed with nonfunctioning PETs) somatostatinoma, producing excessive somatostatin watery diarrhea, hypokalemia-achlorhydria (WDHA) CRHoma, producing excessive corticotropin-releasing hormonse (CRH) calcitoninoma, producing excessive calcitonin GHRHoma, producing excessive growth-hormone-releasing hormone (GHRH) neurotensinoma, producing excessive neurotensin ACTHoma, producing excessive adrenocorticotropic hormone (ACTH) GRFoma, producing excessive Growth hormone-releasing factor (GRF) parathyroid hormone–related peptide tumor rare GEP-NETs o medullary carcinoma of the thyroid o Merkel cell cancer (trabecular cancer) o small-cell lung cancer (SCLC) o large-cell neuroendocrine carcinoma (of the lung) o extrapulmonary small cell carcinomas (ESCC or EPSCC)in general o neuroendocrine carcinoma of the cervix o Multiple Endocrine Neoplasia type 1 (MEN-1 or MEN1) (usually nonfunctioning) (also causing ZES)
