After Site Visit:
1. The team members will complete the Exit Summary Sheet based on a debriefing with the team and
information provided through all observations, interviews, and documentation summary.
Make sure that observers understand that “Overall evidence” asked for at the top of each strategy
implementation page should be an average of the evidence seen of all indicators on that particular
strategy observation page.
Have each team member count up the amount of overall evidence they saw of each strategy and
place the appropriate number after the “degree of evidence” on the Exit Summary Sheet.
2. The team leader will provide a quarterly report to the school.
3. The district completes the web‐based implementation status report.
