In order to provide the greatest amount of assistance and training to the schools, the Louisiana
Department of Education is very interested in observing the progress of the implementation of the
scientifically based research strategies within the school improvement plan. The DIG Team will be at
your school on (insert date).
Additionally, we would very much appreciate your filling out the attached questionnaire and returning it
to the designated team member on (insert day). This is completely anonymous. Do not put your name
on it or turn it in to the office. In order to protect your anonymity, place the questionnaire in the
attached envelope and the designated team member will pick it up from you on the morning of (insert
date).
Input from all teachers is very important to us. Please take the time to complete this questionnaire.
