A typical nonconformance report is shown in Figure 14-1.<br />

14.1.2.1 Stop-Work Order<br />

If corrective actions are insufficient, resolution cannot be reached, or results of prior work are<br />

indeterminate, work may be stopped by a Stop-Work Order. The Stop-Work Order can only be<br />

authorized by the PM or QA PM in writing. If there is a disagreement between these personnel, the<br />

differences will be brought to the attention of succeeding levels of management until resolution is<br />

achieved.<br />

The conditions for which the Stop-Work Order was issued will be described in sufficient detail to allow<br />

proper evaluation of the problems and to affect proper corrective action. Documentation of discussions,<br />

telephone conference notes, or correspondence which describe the actions taken to evaluate the<br />

problems, provide solutions, and verify implementation of solutions will be attached to the Stop-Work<br />

Order and fully referenced in the appropriate spaces. Work will not continue until the Stop-Work Order<br />

has been rescinded by the individual that authorized the order.<br />

14.1.2.2 Cause and Action to Prevent Recurrence<br />

The QA PM will track the NCRs, analyze the corrective actions required, and take the necessary steps to<br />

resolve the causes of the nonconforming conditions in order to prevent recurrence.<br />

2.43 Field Changes<br />

The PM or his/her designee is responsible for all site activities. In this role, the PM at times is required to<br />

adjust the site programs to accommodate project-specific needs. Field changes may be required when the<br />

sample network is changed (i.e., reduction/addition of samples, sampling locations other than those<br />

specified in the FSP, etc.), sampling procedures require modification due to unexpected conditions, or it<br />

is not possible to implement corrective actions for deficient/nonconforming condition. In addition, the<br />

appropriate Contract Officer must be notified prior to the implementation of any changes in the sampling<br />

plans<br />

When it becomes necessary to modify a field program, the FOL or designated field lead will notify the<br />

PM of the anticipated changes and implement the necessary changes. When a change is determined to be<br />

necessary, the initiator of the change will submit a written notification and a copy will be filed in the<br />

project records. If unacceptable, the action taken during the period of deviation will be evaluated in<br />

order to determine the significance of any departure from established program practices and action taken.<br />

The changes in the program are documented on a field change request (FCR) form that is signed by the<br />

initiator and PM. A typical FCR form utilized to document field changes is shown in Figure 14-2. The<br />

FCRs for each project will be numbered serially starting with the number "1".<br />

The PM is responsible for the controlling, tracking, and implementing identified changes. Completed<br />

FCRs are distributed to affected parties and those parties to whom copies of affected plans were<br />

assigned. Completed FCRs are maintained in the project files.<br />
