Conduits through Embankment Damswas utilized in the grout mix. The initial mix was 0.8:1 (water/cement ratio byvolume). When it was determined that no problems were encountered with thegrouting operations, the mix was reduced to 0.7:1. A grout fluidfier was used in thegrout mix. Upon completion of the grouting operations, an ASTM C33 sand filterwas installed at the downstream end of the conduit.Lessons learned• CCTV inspection equipment can be used to identify deteriorated areas withininaccessible conduits.• Expedited dam safety actions require good communication between allinterested parties and agencies involved. • A siphon can be constructed quickly and inexpensively in order to providedownstream irrigation requirements.ReferenceBureau of Reclamation, Pasture Canyon Dam—Outlet Works Abandonment, November2004.B-80
Appendix B—Case HistoriesProject name: Piketberg DamLocation: South AfricaSummary: Failure of an embankment dam by internal erosion resulting fromhydraulic fracture of earthfill adjacent to the outlet conduitIn 1986, Piketburg Dam, a 40-foot high embankment dam was built across a minortributary of the Verlore Vlei River, near the town of Piketberg in the Western Cape,South Africa. The new embankment dam was constructed over an existing dam atthe site to increase storage. Figure B-63 shows a cross section of the embankmentdam.After 5 weeks following construction, during which water was pumped into thereservoir, and when it was almost full, major leakage suddenly appeared at thedownstream toe of the embankment dam near the outlet. Within less than a day, theentire contents of the reservoir had been lost through a cavern adjacent to the outletconduit.Inspection after the event revealed a major tunnel through the entire width of theembankment dam along the outlet conduit. At the time of the inspection, the roofof the tunnel had collapsed over the entry and exit. The center portion of the tunnelbeneath the dam crest, however, remained intact arching almost 33 feet across thetunnel. Large sinkholes were present in the upstream slope of the dam.The embankment material was broadly graded from coarse gravel sizes to clay sizes,typically with a liquid limit of 28 percent and plasticity index of 9. Both residual soil(decomposed phyllite and greywacke) and transported soil were utilized.
