Appendix B—Case HistoriesThe cause of failure of the PVC diversion pipe was not determined. The cause mayhave been the result of the grouting work, poor construction practices, faulty pipe, ora combination of these factors.Lessons learnedInternal inspection of all conduits within an embankment dam should be conductedat the end of construction to ensure that the conduits are not excessively deformedand that they will perform as intended. PVC pipe is not recommended for use inhigh or significant hazard embankment dams, unless it is fully encased in reinforcedcast-in-place concrete.
