Developments since consultation<br />

Out of hospital services are already<br />

in place and plans are in place<br />

within each CCG for 2013/14 (see<br />

Chapter 16). As a result, a<br />

significant proportion of our out of<br />

hospital services will be in place in<br />

2013/14<br />

Out of hospital delivery is<br />

supported across NW London by a<br />

range of transformation<br />

programmes, including primary<br />

care development and integrated<br />

care (see Chapter 8b)<br />

Plans have been developed to<br />

couple the development of out of<br />

hospital services with changes to<br />

acute services. These plans<br />

include ways to monitor delivery of<br />

out of hospital strategies,<br />

timescales for changing acute<br />

provision to 2017/18, and the<br />

mechanism for aligning decisionmaking<br />

with out of hospital delivery<br />

(see Chapter 18 for implementation<br />

plans).<br />
