Integrated: Where possible, care should be integrated between primary and<br />

secondary care, with involvement from social care, to give patients a co-ordinated<br />

service. This integration across organisational boundaries will provide a seamless<br />

experience of care in range of care settings.<br />

The vision was agreed by the Clinical Board and the Programme Board pre-consultation.<br />

9.4.3 Feedback received about the vision during consultation<br />

During consultation we sought feedback about the vision. We wanted to understand if people<br />

agreed with the vision for localising and centralising care and if people had any suggestions<br />

for how it could be improved.<br />

We asked people two questions in our consultation response form. The first question was:<br />

4,595 people answered this question:<br />

43% respondents answering this question agree that some services which are<br />

currently delivered in hospital could be delivered more locally<br />

25% disagree<br />

31% express no view either way<br />

We also asked people to consider the centralisation of services:<br />

Q6. How far do you support or oppose the idea of bringing more healthcare services<br />

together on fewer sites<br />

4,628 people answered this question:<br />

30% support the idea of bringing more healthcare services together on fewer sites<br />

38% oppose<br />

31% had no views either way<br />

In addition to the feedback above, we received responses from organisations addressing the<br />

vision:<br />

“The principles and objectives - to prevent ill health in the first place; to provide easy access<br />

to high quality GPs and their teams; and to support patients with long term conditions and to<br />

“The Council fully understands the case for change from both a service quality and financial<br />

perspectives, and fully supports the direction of travel which will enable people to receive<br />

health and care support in their own homes and in community settings, avoiding the need for<br />

hospital admission unless this is absolutely necessary.”<br />

Royal College of Paediatrics and Child Health<br />

“We strongly support the principle of care provision using integrated care pathways within<br />

the clinical network model, and that outcome measures must drive service improvement.”<br />
