Developments since consultation<br />

Effective integrated care is one<br />

strand of all out of hospital<br />

strategies (including integrated<br />

care pilots covering all eight CCGs<br />

in NW London). Delivery began in<br />

2012/13 and further developments<br />

are planned for 2013/14, including<br />

whole systems integrated care (see<br />

Chapter 8, Section 8.23.2)<br />

Mental health services will be<br />

located on many hub/health centre<br />

and local hospital sites, improving<br />

continuity and consistency of care<br />

(see Chapter 8, Section 8.12)<br />

Independent evaluation of the Inner<br />

NW London Integrated Care Pilot<br />

suggests early successes (see<br />

Chapter 8, Section 8.21.2).<br />

Significant investment in out of<br />

hospital services is planned across<br />

the life of <strong>Shaping</strong> a <strong>healthier</strong><br />

<strong>future</strong> (see Chapter 16)<br />

This investment includes revenue<br />

investment in services and capital<br />

investment in new settings of care.<br />

8.5. Developments since consultation<br />

Since consultation, our plans for the delivery of out of hospital services have developed<br />

further, to both respond to the points raised by the public and to provide more detailed<br />

information about how out of hospital services will be delivered.<br />

This includes:<br />

Understanding further the views of NW London patients and carers regarding<br />

primary care development and the impact of out of hospital services on carers<br />

Developing our plans for workforce and out of hospital estate (including local<br />

hospitals, networks and hubs/health centres) further<br />

Beginning the implementation of out of hospital standards in accordance with our<br />

out of hospital strategies and in concordance with the recommendation<br />

Refining our plans for urgent care centres to ensure they meet the needs of patients<br />

Providing further confidence in the delivery of out of hospital services, including<br />

our investment in out of hospital, performance to date and plans for 2013/14.<br />
