Sample <strong>to</strong>ol availablein English and Spanish at http://riskfac<strong>to</strong>r.cancer.gov/DHQ/)The Diet His<strong>to</strong>ry Questionnaire (DHQ) is a food frequency questionnaire(FFQ) developed by staff at the Risk Fac<strong>to</strong>r Moni<strong>to</strong>ring and Methods Branch(RFMMB) of the National Cancer Institute (NCI). This FFQ consists of 124food items and includes both portion size and dietary supplement questions. Ittakes about 1 hour <strong>to</strong> complete and was designed, based on cognitive researchfi ndings, <strong>to</strong> be easy <strong>to</strong> use.Data show that the DHQ provides reasonable nutrient estimates, and the<strong>to</strong>ol has been validated. The DHQ is available in a variety of file formats,each designed for a specifi c method of capturing the data from paper-basedforms. Review the options below and determine the method that you will use<strong>to</strong> capture the data before downloading and printing copies for distribution.The DHQ and other required files are available on each method’s page. As ofAugust 2005, a web-based version of the DHQ is also available as an optionfor capturing data. The <strong>to</strong>ol is available in both English and Spanish.DHQ data s<strong>to</strong>red in an ASCII text fi le can be analyzed using theDiet*Calcsoftware. Please be aware that although Diet*Calc is available at nocost, there may be costs associated with printing the forms and with scanningor entering the data. There are 3 methods for capturing survey data -- scanning,data entry, and computer-assisted interviews. To date, scanning hasproven <strong>to</strong> be the most effective method for DHQ.
