Information on low cost insurance policiesdesigned especially for students may be obtained from StudentLife, the Student Health & Counseling Center, or the websitewww.csuhealthlink.com.Immunization RequirementsEntering <strong>CSU</strong> students are required to present proof of thefollowing immunizations to the <strong>CSU</strong> campus that they will beattending before the beginning of their first term of enrollment(these are not admission requirements, but shall be required ofstudents as conditions of enrollment in the <strong>CSU</strong>):Measles and RubellaAll new and readmitted students born after January 1, 1957must provide proof of immunization against measles and rubellaprior to enrollment. Submit medical documentation to StudentHealth Services to verify both vaccinations were received sinceage one or obtain the vaccination at an additional chargethrough the SH&CC.Hepatitis BAll new students who will be 18 years of age or younger atthe start of their first term at a <strong>CSU</strong> campus must provide proofof full immunization against Hepatitis B before enrolling. Fullimmunization against Hepatitis B consists of three timed dosesof vaccine over a minimum of four to six months. If you needfurther details or have special circumstances, please consultStudent Health Services at (805) 437-8828.MeningococcusMeningococcal vaccine is not required; however, eachincoming student who will reside in on-campus housing willbe required to return a form to the Housing & ResidentialEducation (H&RE) office verifying that he or she has receivedinformation about meningococcal disease and the availabilityof a vaccine to prevent one from contracting it. Each studentmust also indicate on the form whether he or she has chosen toreceive the vaccination or not. This form is available at the HREoffice, SH&CC or online through the CI website. SLP includesthree programmatic elements: the SEAL Center, Clubs andOrganizations, and the Leadership Certificate Program (currentlyunder development).Student Engagement and Applied Leadership(SEAL) CenterThe SEAL Center is staffed by experienced student leaders andis a location for aspiring and current student leaders to gaininformation about leadership opportunities and trainings oncampus. Various resources for clubs and organizations are alsoavailable in the SEAL Center.Leadership Certification ProgramSLP is currently in the process of developing a multi-phaseleadership program that will provide opportunities for studentsto develop to their fullest leadership potential. Although thesecond and third levels are still under development, the firstlevel of the leadership certificate program is the Back to BasicsLeadership Retreat and is designed to teach aspiring and currentstudent leaders the significance of “going back to the basics” tounderstand self, leadership, team, professionalism, self-discipline,and etiquette.Clubs and OrganizationsClubs and organizations must design and implement programs,events and activities which support and enrich the goals of CI’seducational mission. Involvement in clubs/organizations presentsstudents with the opportunity to broaden their learning, obtainleadership and interpersonal skills, and develop a commitmentto service. Student participation in clubs/organizationsattracts new students to our campus and integrates them intoour CI culture and traditions.
