Epidemiology, transmissi<strong>on</strong> and ec<strong>on</strong>omics <str<strong>on</strong>g>of</str<strong>on</strong>g> Johne’s disease: state <str<strong>on</strong>g>of</str<strong>on</strong>g> knowledge,<br />

knowledge gaps, and possible future directi<strong>on</strong>s<br />

Evidence-based medicine is predicated <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> principle that risks and benefits <str<strong>on</strong>g>of</str<strong>on</strong>g> interventi<strong>on</strong>s can be explicitly<br />

quantified through scientifically-valid studies. These studies can <str<strong>on</strong>g>the</str<strong>on</strong>g>n inform decisi<strong>on</strong>s about <str<strong>on</strong>g>the</str<strong>on</strong>g> ec<strong>on</strong>omic<br />

value <str<strong>on</strong>g>of</str<strong>on</strong>g> interventi<strong>on</strong>s whe<str<strong>on</strong>g>the</str<strong>on</strong>g>r for a single herd or flock, or for nati<strong>on</strong>al and regi<strong>on</strong>al programs. Current<br />

knowledge indicates that <str<strong>on</strong>g>the</str<strong>on</strong>g>re are multiple sources <str<strong>on</strong>g>of</str<strong>on</strong>g> MAP infecti<strong>on</strong> and c<strong>on</strong>sequently, <str<strong>on</strong>g>the</str<strong>on</strong>g> recommendati<strong>on</strong><br />

lists for producers to c<strong>on</strong>trol Johne’s disease are l<strong>on</strong>g and <str<strong>on</strong>g>of</str<strong>on</strong>g>ten unprioritized. There are few published<br />

randomized c<strong>on</strong>trolled trials (RCT) or cohort studies assessing <str<strong>on</strong>g>the</str<strong>on</strong>g> effects, let al<strong>on</strong>e <str<strong>on</strong>g>the</str<strong>on</strong>g> cost-benefits,<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> interventi<strong>on</strong>s. Evidence from <str<strong>on</strong>g>the</str<strong>on</strong>g> U.S. Johne’s Disease Dem<strong>on</strong>strati<strong>on</strong> Herd Project and experiences<br />

elsewhere indicate that effective c<strong>on</strong>trol is possible, but eradicati<strong>on</strong> is unlikely in intensive producti<strong>on</strong> systems<br />

such as large dairy herds. Data <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> benefit-cost ratio for individual or multiple interventi<strong>on</strong>s or entire c<strong>on</strong>trol<br />

program are sparse. This is a critical need for this informati<strong>on</strong> for dairy herds because Johne’s disease is<br />

typically not a high priority disease from a financial perspective. This may change with increasing pressure<br />

from public health authorities to mitigate risks from dairy and meat products. Any expectati<strong>on</strong> that interventi<strong>on</strong>s<br />

that are applied <strong>on</strong> farm or al<strong>on</strong>g <str<strong>on</strong>g>the</str<strong>on</strong>g> producti<strong>on</strong> chain can ever be linked to a reducti<strong>on</strong> in Crohn’s disease<br />

incidence is very unrealistic.<br />

Investigati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> epidemiology, transmissi<strong>on</strong> and ec<strong>on</strong>omics <str<strong>on</strong>g>of</str<strong>on</strong>g> Johne’s disease <str<strong>on</strong>g>of</str<strong>on</strong>g>fers numerous challenges,<br />

which can be broadly categorized as biological, financial and methodological. I would like to focus <strong>on</strong><br />

<str<strong>on</strong>g>the</str<strong>on</strong>g> latter and <str<strong>on</strong>g>of</str<strong>on</strong>g>fer some perspectives <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> future. First, <str<strong>on</strong>g>the</str<strong>on</strong>g>re has been inadequate use <str<strong>on</strong>g>of</str<strong>on</strong>g> l<strong>on</strong>gitudinal<br />

designs, especially RCT, to evaluate interventi<strong>on</strong>s and as a c<strong>on</strong>sequence, <str<strong>on</strong>g>the</str<strong>on</strong>g>re are no systematic reviews<br />

<str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> effects and ec<strong>on</strong>omics <str<strong>on</strong>g>of</str<strong>on</strong>g> interventi<strong>on</strong>s. Cost and feasibility are <str<strong>on</strong>g>the</str<strong>on</strong>g> key limitati<strong>on</strong>s. Sec<strong>on</strong>d, <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

lack <str<strong>on</strong>g>of</str<strong>on</strong>g> uniformly-applied quality standards for publicati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> risk factor studies is symptomatic <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> reality<br />

that we lag behind our human health colleagues (e.g. CONSORT and STROBE in human health). Casec<strong>on</strong>trol<br />

and cross-secti<strong>on</strong>al studies have outlived <str<strong>on</strong>g>the</str<strong>on</strong>g>ir usefulness for bovine and ovine paratuberculosis.<br />

Third, statistical methods exist to incorporate prior knowledge into risk factor analyses and adjust for n<strong>on</strong>differential<br />

misclassificati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> outcome (e.g. ELISA, culture or PCR-based test results) but <str<strong>on</strong>g>the</str<strong>on</strong>g>se methods<br />

are infrequently used. The challenge in <str<strong>on</strong>g>the</str<strong>on</strong>g> next 5 years is to design, finance and implement well-planned<br />

l<strong>on</strong>gitudinal studies to answer critical research questi<strong>on</strong>s through networks and programs such as JDIP, JDRC<br />

and ParaTBTools.<br />
