Two Worlds <strong>on</strong> a Collisi<strong>on</strong> Course<br />

ABSTRACT<br />

The veterinary and medical perspectives as to <str<strong>on</strong>g>the</str<strong>on</strong>g> zo<strong>on</strong>ic potential <str<strong>on</strong>g>of</str<strong>on</strong>g> Mycobacterium avium<br />

subspecies paratuberculosis are <strong>on</strong> a collisi<strong>on</strong> course.<br />

INTRODUCTION<br />

There has been a l<strong>on</strong>g simmering c<strong>on</strong>troversy between <str<strong>on</strong>g>the</str<strong>on</strong>g> worlds <str<strong>on</strong>g>of</str<strong>on</strong>g> medicine and<br />

veterinary sciences as to whe<str<strong>on</strong>g>the</str<strong>on</strong>g>r Mycobacterium avium subspecies paratuberculosis (Map)<br />

and related genomic variants c<strong>on</strong>stitute a threat to <str<strong>on</strong>g>the</str<strong>on</strong>g> public welfare.<br />

The <str<strong>on</strong>g>the</str<strong>on</strong>g>oretical bridge linking <str<strong>on</strong>g>the</str<strong>on</strong>g> two worlds is <str<strong>on</strong>g>the</str<strong>on</strong>g> failure <str<strong>on</strong>g>of</str<strong>on</strong>g> pasteurizati<strong>on</strong> to completely<br />

destroy Map and thus enter <str<strong>on</strong>g>the</str<strong>on</strong>g> human food chain and evidence that Map has <str<strong>on</strong>g>the</str<strong>on</strong>g> potential to<br />

functi<strong>on</strong> as a zo<strong>on</strong>ic pathogen.<br />

HISTORICAL PERSPECTIVE<br />

In 1999, <str<strong>on</strong>g>the</str<strong>on</strong>g> Nati<strong>on</strong>al Institute <str<strong>on</strong>g>of</str<strong>on</strong>g> Allergy and Infectious Diseases (NIAID) published its<br />

research agenda in which it targeted an infectious cause <str<strong>on</strong>g>of</str<strong>on</strong>g> Crohn’s disease. The U.S.<br />

C<strong>on</strong>gress elected not to fund this initiative.<br />

In 2000, <str<strong>on</strong>g>the</str<strong>on</strong>g> Centers for Disease C<strong>on</strong>trol and Preventi<strong>on</strong> (CDC) issued a working document<br />

related to identifying risk factors for human zo<strong>on</strong>ic infecti<strong>on</strong>s including Crohn’s disease. The<br />

project was not funded.<br />

In 2000, <str<strong>on</strong>g>the</str<strong>on</strong>g> United States Department <str<strong>on</strong>g>of</str<strong>on</strong>g> Agriculture introduced and advocated <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

implementati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> U.S. Voluntary Johne’s Disease Herd Program Herd Status Program<br />

(An<strong>on</strong>., 2000b:2000c; 2000d; 2005). The U.S. Voluntary Johne’s Disease Herd Program<br />

Herd Status Program provided voluntary guidelines for herd risk assessment and identified<br />

management practices designed to reduce <str<strong>on</strong>g>the</str<strong>on</strong>g> prevalence <str<strong>on</strong>g>of</str<strong>on</strong>g> infecti<strong>on</strong>. In <str<strong>on</strong>g>the</str<strong>on</strong>g> same<br />

timeframe, revisi<strong>on</strong>s to parts 71 and 80 <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> Code <str<strong>on</strong>g>of</str<strong>on</strong>g> Federal Regulati<strong>on</strong> (CFR) were made<br />

that restricted <str<strong>on</strong>g>the</str<strong>on</strong>g> interstate transportati<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> Map-infected animals except to recognized<br />

slaughter establishments (USDA 2000).<br />

In June 2001, <str<strong>on</strong>g>the</str<strong>on</strong>g> United Kingdom Food Standard Agency issued its report for food<br />

standards. The c<strong>on</strong>clusi<strong>on</strong> statement states “There is undoubtedly sufficient cause for<br />

c<strong>on</strong>cern (relative to Map as being <str<strong>on</strong>g>the</str<strong>on</strong>g> cause <str<strong>on</strong>g>of</str<strong>on</strong>g> Crohn’s disease) for fur<str<strong>on</strong>g>the</str<strong>on</strong>g>r acti<strong>on</strong> to be taken<br />

urgently to determine what <str<strong>on</strong>g>the</str<strong>on</strong>g> available data means …… This questi<strong>on</strong> can be divided<br />

into two areas: What acti<strong>on</strong> should be taken to reduce exposure to Map even though <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

causal link is not established; and what acti<strong>on</strong> can be taken to increase <str<strong>on</strong>g>the</str<strong>on</strong>g> knowledge base<br />

so that future decisi<strong>on</strong>s may be based up<strong>on</strong> more informati<strong>on</strong> (1).”<br />

In April 2002, USDA-APHIS published <str<strong>on</strong>g>the</str<strong>on</strong>g> Uniform Program Standards for <str<strong>on</strong>g>the</str<strong>on</strong>g> Voluntary<br />

Bovine Johne’s Disease C<strong>on</strong>trol Program and instituted a 5 year Johne’s Disease Preventi<strong>on</strong><br />

Dairy Herd Dem<strong>on</strong>strati<strong>on</strong> Program to develop voluntary guidelines designed to reduce Map<br />

herd prevalence.<br />

In 2003, The Nati<strong>on</strong>al Academy <str<strong>on</strong>g>of</str<strong>on</strong>g> Press published <str<strong>on</strong>g>the</str<strong>on</strong>g> nati<strong>on</strong>al Academy <str<strong>on</strong>g>of</str<strong>on</strong>g> Sciences’<br />

analysis <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> Crohn’s disease/Map c<strong>on</strong>troversy “There remains insufficient evidence to<br />

prove or disprove that Mycobacterium avium subsp. paratuberculosis is a cause <str<strong>on</strong>g>of</str<strong>on</strong>g> some or<br />

all cases <str<strong>on</strong>g>of</str<strong>on</strong>g> Crohn’s disease. … A causal link between Map and Crohn’s disease remains a<br />

plausible hypo<str<strong>on</strong>g>the</str<strong>on</strong>g>sis….”<br />
