RESULTS<br />

In 601 (29%) <str<strong>on</strong>g>of</str<strong>on</strong>g> 2050 samples, <str<strong>on</strong>g>the</str<strong>on</strong>g> ESP-system indicated growth <str<strong>on</strong>g>of</str<strong>on</strong>g> Map based <strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g> optical<br />

signal which indicates <str<strong>on</strong>g>the</str<strong>on</strong>g> reducti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> gas-pressure in culture flasks. However, <strong>on</strong>ly 29% <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

<str<strong>on</strong>g>the</str<strong>on</strong>g>se 601 samples could be c<strong>on</strong>firmed using both <str<strong>on</strong>g>the</str<strong>on</strong>g> Ziehl-Neelsen staining <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> culture<br />

material and <str<strong>on</strong>g>the</str<strong>on</strong>g> IS900-PCR. Moreover, in 8% <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> 2050 samples <str<strong>on</strong>g>the</str<strong>on</strong>g> ESP-system did not<br />

indicate growth <str<strong>on</strong>g>of</str<strong>on</strong>g> Map whilst <str<strong>on</strong>g>the</str<strong>on</strong>g> samples were Ziehl-Neelsen-positive and IS900-PCRpositive.<br />

Overall, 340 <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> 2050 samples (17%) cultured in <str<strong>on</strong>g>the</str<strong>on</strong>g> ESP liquid culture were ZNpositive<br />

and PCR-positive.<br />

In 352 (17%) <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> 2050 samples, suspected col<strong>on</strong>ies were found in <str<strong>on</strong>g>the</str<strong>on</strong>g> LJ-culture that were<br />

subsequently c<strong>on</strong>firmed as Map using <str<strong>on</strong>g>the</str<strong>on</strong>g> IS900-PCR. Approximately 60% <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g>se samples<br />

were found positive at 8 weeks <str<strong>on</strong>g>of</str<strong>on</strong>g> incubati<strong>on</strong>.<br />

Overall, 254 samples (12%) were positive by both ESP liquid culture (defined as acid-fast<br />

bacilli using <str<strong>on</strong>g>the</str<strong>on</strong>g> ZN-staining and PCR-positive as c<strong>on</strong>firmati<strong>on</strong>) and <str<strong>on</strong>g>the</str<strong>on</strong>g> LJ solid culture<br />

method, and 1612 (79%) were negative by both methods (Table 1). A fur<str<strong>on</strong>g>the</str<strong>on</strong>g>r 86 samples<br />

(4%) were positive by ESP culture <strong>on</strong>ly, whilst 98 samples (5%) were positive by LJ culture<br />

<strong>on</strong>ly. Agreement bey<strong>on</strong>d chance between both methods was reas<strong>on</strong>able (� = 0.68; 0,64-<br />

0,72).<br />

DISCUSSION AND CONCLUSION<br />

In total 2050 samples were investigated in-parallel with both a solid phase culture method<br />

using Löwenstein-Jensen media and <str<strong>on</strong>g>the</str<strong>on</strong>g> liquid media ESP TREK system.<br />

The detecti<strong>on</strong> rates <str<strong>on</strong>g>of</str<strong>on</strong>g> both methods were comparable for <str<strong>on</strong>g>the</str<strong>on</strong>g> detecti<strong>on</strong> <str<strong>on</strong>g>of</str<strong>on</strong>g> Map in feces.<br />

<str<strong>on</strong>g>the</str<strong>on</strong>g>re was a reas<strong>on</strong>able agreement between both methods. However, to obtain a comparable<br />

detecti<strong>on</strong> rate with <str<strong>on</strong>g>the</str<strong>on</strong>g> ESP system, it is necessary to test all samples after 6 weeks <str<strong>on</strong>g>of</str<strong>on</strong>g><br />

incubati<strong>on</strong> using <str<strong>on</strong>g>the</str<strong>on</strong>g> Ziehl-Neelsen staining and IS900-PCR, which adds c<strong>on</strong>siderably to <str<strong>on</strong>g>the</str<strong>on</strong>g><br />

costs <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> system. The usage <str<strong>on</strong>g>of</str<strong>on</strong>g> <strong>on</strong>ly <str<strong>on</strong>g>the</str<strong>on</strong>g> optical signal <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> ESP system leads to both<br />

false-positive and false-negative results; <str<strong>on</strong>g>the</str<strong>on</strong>g>refore a fur<str<strong>on</strong>g>the</str<strong>on</strong>g>r c<strong>on</strong>firmati<strong>on</strong> is to be advised.<br />

The advantage <str<strong>on</strong>g>of</str<strong>on</strong>g> <str<strong>on</strong>g>the</str<strong>on</strong>g> ESP culture system is a c<strong>on</strong>siderable lower time-to-detecti<strong>on</strong> <str<strong>on</strong>g>the</str<strong>on</strong>g>n with<br />

<str<strong>on</strong>g>the</str<strong>on</strong>g> c<strong>on</strong>venti<strong>on</strong>al culture method <strong>on</strong> Löwenstein-Jensen media.<br />
