Ad oggi, <strong>per</strong>ò, non esistono ancora terapie agoniste o antagoniste <strong>per</strong> cocaina, amfetamine e cannabis. L’impossibilità di dare sollievo con trattamenti farmacologici appropriati a soggetti che abusano di sostanze quali cocaina, amfetamine e cannabis ha spinto i ricercatori a cercare trattamenti efficaci anche <strong>per</strong> <strong>le</strong> <strong>dipendenze</strong> da queste sostanze (Serpelloni et al. 2008). L’esposizione ripetuta al<strong>le</strong> droghe può indurre adattamenti neuronali a lungo termine in alcuni sistemi. Questi neuroadattamenti sono in parte associati all’alterata attività della dopamina nel circuito mesocorticolimbico (Hyman et al. 2006, Vanderschuren & Kalivas 2000) e portano ad un’alterazione della neurotrasmissione del glutammato (Wolf et al. 2004) e dell’eccitabilità cortica<strong>le</strong>. In questo capitolo si vuo<strong>le</strong> offrire una panoramica sulla situazione attua<strong>le</strong> della ricerca nell’ambito dello studio e del trattamento del<strong>le</strong> <strong>dipendenze</strong> con la stimolazione cerebra<strong>le</strong>. Le tecniche non invasive di stimolazione cerebra<strong>le</strong> che sembrano avere <strong>le</strong> potenzialità <strong>per</strong> apportare benefici nel trattamento della dipendenza da sostanze sono la Stimolazione Magnetica Transcranica (TMS, dall’ing<strong>le</strong>se Transcranial Magnetic Stimulation), ampiamente descritte nel capitolo precedente, e la Stimolazione Transcranica a Corrente Continua (tDCS, dall’ing<strong>le</strong>se Transcranial direct current stimulation). APPLICAZIONI DELLA TMS NELL’AMBITO DELLA RICERCA SULLE TOSSICODIPENDENZE La Stimolazione Magnetica Transcranica è una tecnica relativamente nuova nel panorama del<strong>le</strong> metodiche a disposizione della s<strong>per</strong>imentazione psicologica. Essa consente di stimolare o inibire la corteccia cerebra<strong>le</strong> in modo non invasivo (figura 1). Come <strong>per</strong> altre patologie, anche nell’ambito del<strong>le</strong> tossico<strong>dipendenze</strong> vanno distinti gli studi con Stimolazione Magnetica Transcranica più strettamente dedicati alla ricerca da quelli specificamente rivolti al trattamento. Indubbiamente interessanti sono gli studi con TMS sull’inibizione cortica<strong>le</strong> nella corteccia cerebra<strong>le</strong> di soggetti con dipendenza da sostanze.
