sarcomatoïde verandering, waardoor gezocht moet worden naar gebieden met herkenbare
differentiatie (het clearcell aspect). Deze schaarse gebieden zijn dan wel diagnostisch voor
het subtype. Papillaire niercelcarcinoom tonen meestal een diffuus papillair beeld (≥ 75%) en
zijn als zodanig goed te herkennen. Aanwezigheid van histiocytaire schuimcellen in de
stromakernen kunnen helpen bij het stellen van de diagnose. Focale papillaire formaties op
zich zijn onvoldoende voor de diagnose papillair niercelcarcinoom. Zeldzaam zijn focale
clearcell veranderingen in het papillaire niercelcarcinoom. Is de tumor geheel van clearcell
aspect met papillaire groeiwijze, dan blijkt uit cytogenetisch onderzoek dit toch als clearcell
niercelcarcinoom te moeten worden beschouwd (Salama 2003).
Het chromofobe subtype is zeldzamer en een lastiger te onderscheiden subtype. Belangrijke
criteria voor de diagnose zijn monotoon solide aspect met toename in cel- en kerngrootte
langs smalle fibrovasculaire septa en de perinucleaire halo. Hale’s ijzerkleuring blijkt in de
praktijk weinig bruikbaar. Er bestaat een eosinofiele variant die goed onderscheiden moet
worden van het oncocytoom, een benigne tumor.
Indien subtypering niet mogelijk lijkt is niercelcarcinoom, unclassified, een geaccepteerd
alternatief.
Ondersteunende immunohistologie is meestal niet nodig, maar kan soms helpen in lastige
gevallen (zie tabel 1.2). Met nadruk wordt hier opgemerkt dat de interpretatie met
voortschrijdend inzicht dient te geschieden.
Aanbeveling
De term Grawitz tumor is obsoleet en dient vervangen te worden door niercelcarcinoom met
subtype aanduiding volgens de WHO 2004.
