(3) Request roll-offs or drums to dispose of ISW;
(4) Repair or maintain equipment that contains or stores ISW;
(5) Discard fluorescent lamps or lead acid/nickel cadmium/mercury/silver batteries;
(6) Supervise employees who have any of the above responsibilities; or
(7) Serve as a Facility Manager in a facility where ISW is generated or stored.
3.21 Records and Forms
The EO shall retain all submitted JF1104s.

Find the specific waste type, activity, or storage device in the left column below. The
requirements for notification, labeling, storage areas, accumulation, and waste pickup are
found in the row of the table associated with that waste, activity, or device. For example,
asbestos collected in a roll-off container shall meet the requirements found in Sections 3.4.1,
3.4.2, 3.6.5, 3.6.6, 3.7.3, 3.9.3, 3.10.3. Sections 3.2 and 3.4 apply to anyone generating and
collecting wastes. New wastes, altered wastes, and special packaging may require review and
approval by the EO using procedures outlined in Chapter 2 Environmental Planning.
