sti le <strong>di</strong>verse specie <strong>di</strong> arsenico sono risultate tutte significativamente<br />
correlate tra loro. È risultata una significativa eliminazione <strong>di</strong> arsenobetaina<br />
associata all’alto consumo <strong>di</strong> molluschi e una significativa eliminazione<br />
<strong>di</strong> arsenobetaina, MMA, iAs e As totale associata al consumo<br />
<strong>di</strong> molluschi e/o crostacei nei tre giorni prec<strong>ed</strong>enti la raccolta<br />
<strong>del</strong>le urine. L’analisi <strong>del</strong>la regressione applicata su tutte le variabili continue<br />
ha mostrato negli esposti e nei non esposti una <strong>di</strong>pendenza significativa<br />
<strong>del</strong>l’eliminazione <strong>di</strong> arsenobetaina, iAs e As totale dal consumo<br />
<strong>del</strong>l’ultimo pasto a base <strong>di</strong> crostacei e/o molluschi nei tre giorni prec<strong>ed</strong>enti<br />
la raccolta <strong>del</strong>le urine.<br />
DISCUSSIONE<br />
I risultati <strong>del</strong>la ricerca sembrano in<strong>di</strong>care, in accordo con i dati <strong>di</strong><br />
letteratura, che una <strong>di</strong>eta a base <strong>di</strong> crostacei e/o molluschi, consumati<br />
tre giorni prima <strong>del</strong>la raccolta <strong>del</strong>le urine, sia in grado <strong>di</strong> con<strong>di</strong>zionare<br />
l’eliminazione urinaria non solo <strong>di</strong> As totale e arsenobetaina, ma anche<br />
<strong>del</strong>l’iAs determinato me<strong>di</strong>ante spettrofotometria in assorbimento atomico<br />
(As inorganico+MMA+DMA) (3, 4, 7). Crostacei e molluschi,<br />
infatti, tendono ad assumere l’arsenico inorganico presente nelle acque<br />
e nei se<strong>di</strong>menti marini e a trasformarlo e ad accumularlo in forme organiche.<br />
Al riguardo numerosi stu<strong>di</strong> <strong>di</strong> speciazione condotti recentemente<br />
sui prodotti ittici hanno evidenziato come in crostacei e molluschi,<br />
oltre alle specie organiche <strong>del</strong>l’arsenico, siano presenti anche<br />
forme inorganiche in percentuale variabile dall’1 al 41% rispetto all’arsenico<br />
totale (8).<br />
Ulteriori stu<strong>di</strong> andrebbero condotti nell’uomo al fine <strong>di</strong> comprendere<br />
se l’aumento <strong>del</strong>l’iAs urinario dosato con la spettrofotometria in assorbimento<br />
atomico sia da attribuirsi all’incremento <strong>del</strong>l’MMA o <strong>del</strong> DMA derivato<br />
dal metabolismo <strong>del</strong>l’arsenico inorganico o, come riportato in letteratura,<br />
dal catabolismo <strong>di</strong> composti organoarsenicali presenti nei prodotti<br />
ittici oppure sia da attribuirsi all’aumentata escrezione <strong>di</strong> arsenico<br />
inorganico assunto con i crostacei e molluschi (5).<br />

EFFECTS OF ENDOCRINE DISRUPTERS(EDC) ON ENDOCRINE<br />
SYSTEMS AND REPRODUCTIVE HEALTH<br />

ABSTRACT. Our study had the following aims: to verify retrospectively<br />
whether exposure to EDCs is associat<strong>ed</strong> with menstrual <strong>di</strong>sorders<br />
(polymenorrhea, oligomenorrhea, amenorrhea); to verify whether<br />
exposure to EDCs is associat<strong>ed</strong> with loss of fertility, as in<strong>di</strong>cat<strong>ed</strong><br />
particularly by a prolongation of time to pregnancy; to verify whether<br />
there is a correlation between exposure to EDCs and the incidence<br />
of endometriosis. This epidemiologic study enroll<strong>ed</strong> 80 women<br />
atten<strong>di</strong>ng the gynecological outpatient clinics at the Policlinico<br />
Umberto I, in Rome. A specifically design<strong>ed</strong> questionnaire was us<strong>ed</strong><br />
to collect information on their home environment, <strong>di</strong>etary habits,<br />
recreations and other activities outside work. The questionnaire ask<strong>ed</strong><br />
for details of their current and previous jobs, so as to draw a complete<br />
occupational picture and assess any types of risk they had been expos<strong>ed</strong><br />
to that might have had repercussions on their health; from analysis<br />
of extrapolat<strong>ed</strong> data it seems there may well be an association between<br />
exposure to EDCs and some endocrine pathologies in women. Celiac<br />
<strong>di</strong>sease gave an interesting fin<strong>di</strong>ng. One outstan<strong>di</strong>ng ne<strong>ed</strong> is for<br />
biomarkers of susceptibility and exposure, especially since these<br />
substances are so widespread in the environment and in food, a factor<br />
that limits epidemiological stu<strong>di</strong>es.<br />
INTRODUZIONE<br />
È noto da oltre un decennio come sostanze chimiche oggi largamente<br />
presenti nell’ambiente <strong>di</strong> vita e <strong>di</strong> lavoro possano produrre effetti negativi<br />
sulla funzione endocrina e riproduttiva, anche nella specie umana. Queste<br />
sostanze agiscono sia alterando <strong>di</strong>rettamente la funzione endocrina a<br />
causa <strong>del</strong>la loro affinità per i recettori ormonale, sia attraverso un effetto<br />
<strong>di</strong> tipo tossico sul sistema immunitario, sia nell’uomo che nella donna.<br />
Un esempio classico è fornito dalle <strong>di</strong>ossine che determinano nella donna<br />
gravida incremento <strong>del</strong>l’aborto spontaneo, mentre nel neonato (tramite il<br />
latte materno) possono provocare atrofia <strong>del</strong> tessuto linfatico con ritar<strong>di</strong><br />
<strong>di</strong> crescita <strong>ed</strong> altre anomalie. Un agente chimico può alterare i <strong>di</strong>versi processi<br />
cellulari: <strong>di</strong>fferenziazione, mitosi, meiosi, apoptosi, riparazione <strong>del</strong><br />
DNA, e può produrre effetti in<strong>di</strong>retti, mo<strong>di</strong>ficando la produzione ormonale<br />
endogena. Si è molto <strong>di</strong>scusso <strong>del</strong>l’effetto <strong>di</strong> questi “<strong>di</strong>struttori endocrini”<br />
sulla funzione riproduttiva maschile <strong>ed</strong> in particolare sulla spermatogenesi<br />
e sull’incidenza <strong>di</strong> tumori testicolari. Nella donna questi effetti<br />
sembrano più sfumati e per molti manca la prova che alle concentrazioni<br />
<strong>di</strong> norma rilevate, vi sia un chiaro effetto negativo, sia pure su particolari<br />
sottogruppi a rischio. Alla luce <strong>di</strong> questa analisi dei rischi lo stu<strong>di</strong>o<br />
si propone <strong>di</strong> verificare in modo retrospettivo se l’esposizione ad<br />
agenti che rientrano nella categoria degli interferenti endocrini sia o meno<br />
associata a: <strong>di</strong>sor<strong>di</strong>ni <strong>del</strong> ciclo mestruale (polimenorrea, oligomenorrea,<br />
amenorrea), ad ipofertilità con aumento <strong>del</strong> “time to pregnancy” e ad aumento<br />
<strong>del</strong>l’incidenza <strong>di</strong> endometriosi.<br />
MATERIALI E METODI<br />
Lo stu<strong>di</strong>o epidemiologico è stato effettuato arruolando 80 donne (età<br />
me<strong>di</strong>a 34aa) afferenti agli ambulatori <strong>del</strong> Dipartimento <strong>di</strong> Scienze Ginecologiche<br />
<strong>del</strong> Policlinico Umberto I <strong>di</strong> Roma che sono state <strong>di</strong>vise in tre<br />
gruppi, in relazione alla patologia per la quale si erano sottoposte a con-
