BlueCard®
Benefits and Eligibility You may contact the BlueCard eligibility number for benefits and
eligibility 1-800-676-BLUE (2583) for other plans’ members. If
the automated system on the BlueCard eligibility line cannot
identify the three digit alpha prefix that is being stated, after the
second failed attempt, you will automatically be transferred to a
BlueCard agent. The BlueCard agent will need one of the
following answers to be accurate in order to validate:
BlueCard Preferred
Provider Organization
(PPO)
1. Alpha prefix
2. Plan code
3. Name of Blue Cross plan
4. Employee name
5. State in which the member’s plan is located (can be found on
the back of the member’s ID card)
You will be asked for the alpha prefix on the ID card and will then
be transferred to the member’s plan. They will provide the
information requested.
You may also submit a 270 request via provider web self service
for benefits and eligibility. In addition, you could send an
electronic request (EDI) via Availity, but you must register to do
that. * Be sure to include:
1. ID number, including alpha prefix
2. Patient first and last name
3. Date of birth
* Availity registration information: availity.com.
The BlueCard PPO program is a national program that offers
members the PPO level of benefits when outside their Blue plan
area and they obtain services from a physician or hospital
designated as a PPO provider.
You will immediately recognize these PPO members by the
special “PPO in a suitcase” logo on their ID card.
Blue Cross utilizes the Aware® provider network as our BlueCard
PPO network. Members can access information about providers in
this network via the toll free number: 1-800-810-BLUE (2583) or
on the BlueCard website, bcbs.com.
