Coding Policies and Guidelines (Modifiers)
The national HCPCS Panel developed several modifiers that could
be defined by the various state Medicaid agencies. The Minnesota
Department of Human Services has defined these as follows.
Each modifier has more than one definition dependent on what
service it is appended to or the program affected. The modifiers are
generally informational only and, with the exception of –U7,
applicable primarily to services for our PMAP and MNCare
subscribers.
