Follow the same guidelines for skilled nursing facility care as for
in-hospital medical care above. Use codes 99304-99306 for
medical care on the day of admission to a skilled nursing facility.
For follow-up care, use codes 99307-99310. The place-of-service
code is 31. Claims for skilled nursing care require admit and
discharge dates.
Discharge day management codes 99315 or 99316 are used to
report the total duration of time spent by a physician for the final
nursing facility discharge of a patient.

“0022.”
Loop 2400, SV202 Three (3) character RUG code with two (2)
character assessment type indicator.
Total of five (5) characters; for example
“RVX01.”
Loop 2400, DTP03 The service date of the assessment. Service
date must fall within statement covered
period.
Loop 2400, SV205 The number of days that apply to the RUG
code in FL 44.
Loop 2400, SV203 Zero-fill this form locator; for example “0 |
00.”
