Day Treatment Submit one unit of service per hour for day treatment programs for
behavioral health diagnoses (H2012). Include the actual time of
therapy on the claim also. Refer to the Behavioral Health section
for additional information.
Diabetic Education Blue Cross recommends submission of HCPCS codes G0108 and
G0109 for a diabetes education program that meets the National
Standards for Diabetes Self-Management Education and obtains
recognition status from the American Diabetes Association (ADA)
or program accreditation from the American Association of
Diabetes Educators (AADE); however, program recognition or
accreditation does not guarantee reimbursement of treatment
which includes Diabetes Self-Management Training, services must
be within the scope of an individual practitioner’s registration,
license and training.
