Blue Cross and Blue Shield of Minnesota Provider Policy and Procedure Manual (03/23/12) Coding Policies and Guidelines (Laboratory) Stat Lab Charges Charges for stat laboratory requests (S3600 and S3601) are not allowed. If submitted, they will deny as provider liability. Repeat Lab Services Repeat services require modifier 91 (not 59), unless the narrative supports submission of multiple units. Genetic Testing Modifiers Lab Billed through the BlueCard® Program Genetic coding modifiers (0A-9Z) are developed and maintained by the AMA. These modifiers are required when reporting a genetic lab procedure, as appropriate. Blue plans* may contract with providers outside of their exclusive service area for services provided to local and BlueCard members within their own service area for independent clinical lab services. Blue plans may not contract for such services for their members who receive services outside of their service area. Lab Providers who perform lab services should file the claim to the Blue plan in whose service area the specimen was drawn. The claim will be reimbursed based on provider’s participation status with that Blue plan. * Each Blue plan is an independent licensee of the Blue Cross and Blue Shield Association 11-7
