language in effect at the time services were rendered. Check for
patient benefits before services are rendered.
The following groups do not provide for coverage of E-Care visits:
Federal Employee Program (FEP)
Medicare Supplements
PMAP/MNCARE
Some self-insured groups
Ear Wax Removal Ear wax removal (removal of impacted cerumen) is by CPT
definition a “separate procedure”. Codes designated as separate
procedures should not be reported in addition to the code for the
total procedure or service of which it is considered an integral
component. The denial however, may vary depending on what
code(s) is billed with 69210. For example, code 69210 will deny as
incidental to audiometry evaluation and speech recognition testing,
but if billed with an office visit, the E/M will be denied as
incidental to 69210.
G0101 G0101 (cervical or vaginal cancer screening; pelvic and clinical
breast examination) is an accepted code. However, G0101 will
deny if billed in conjunction with an E/M service.
