The following physical medicine codes require a physician or
therapist to be in constant attendance. Submit the following CPT
codes for physical therapy services:
Code Units of Service
97010-97028, 97039 1 unit for each modality
97032-97036 1 unit for each 15 minutes
The following codes should be used for physical therapy
procedures:

97110- 97124 1 unit for each 15 minutes.
Example:
Coding for therapeutic exercises, 50 minutes:

For procedures that include time increments, over 50 percent of the
indicated time must be rendered and documented in order to bill an
additional unit. In the above example, only three units may be
submitted – three units for the first 45 minutes. The remaining five
minutes may not be reported because it is less than 50 percent of
the 15-minute code increment.
Additional physical therapy codes 97140-97542 and 97597-97606
should be used as defined in CPT.
