Prior Authorization Requirements Coding Policies and Guidelines (Durable Medical Equipment) Fax all prior authorization requests for Blue Cross subscribers using the Prior Authorization Request for DME form (F5893-R02) to (651) 662-2810. Blue Cross will approve or deny prior authorization requests based on a subscriber's contract benefits and the criteria defined in applicable medical policies. Prior authorization requests should be submitted by the DME supplier who will be providing the equipment and should include the appropriate HCPCS code(s). For questions about prior authorizations, call (651) 662-5270 or 1-800-528-0934 (choose option 2, then option 4). Providers can also fax prior authorization questions to (651) 662-2810. Subscribers should call the number on the back of their subscriber ID card for customer service if they have questions. Coding Policies and Guidelines (Durable Medical Equipment) Ineligible Items The following lists of some items that are considered ineligible DME. There is no need to submit prior authorization requests for ineligible items. Abdominal support belts for pregnant women Adaptive eating equipment Air conditioners Air filters Back huggers Balls for therapy Bedpans and urinals Biofeedback device, purchase Blood pressure cuffs and accessories Car seats Computer software & hardware Copes scoliosis brace total recovery program Croup tent Cryocuff (icing device) Drionic devices (sweating devices) 11-4 Note: This is not an all-inclusive list.
