improvement and local learning.
Reports to Corporate Quality,
Performance and Risk Forum.
Receives reports on progress towards
achieving the Information Governance
Toolkit (IGT) and approves the yearly IGT
submission.
Regularly monitors Information
Governance objectives and information
risks and incidents, including the
Caldecott Issues Log, to ensure
appropriate actions are undertaken and
lessons are learnt.
Reports to the Clinical Governance and
Clinical Risk Committee.
Internal Audit Devises a yearly risk-based Internal Audit
Plan based on the Assurance
Framework to provide external
assurances to the Audit Committee and
then the Trust Board.
Clinical Audit Clinical audit is the assessment of the
processes and/or the outcome of care.
Its aim is to stimulate and support national
and local quality improvement
interventions and, through re-auditing to
assess the impact of such interventions.
5BP in full demonstrates this through the
clinical audit calendar.
Outcomes from clinical audit are reported
to Clinical Effectiveness Panel and via the
Research and Audit Forum for review of
recommendations and action plans.
