Section 2: Surveillance and Laboratory Testing<br />

o Develop a HERDS reporting system for hospitals to report daily aggregate data on<br />

the number of suspected and confirmed influenza-associated deaths. It is<br />

anticipated that this electronic reporting system will be the primary method to<br />

collect daily data necessary to monitor the mortality of the pandemic.<br />

o Provide mortality data to CDC as needed to help guide national response<br />

measures. Case definitions and reporting procedures will be communicated by<br />

CDC via Epi-X.<br />

• Participate in national and international surveillance activities as indicated.<br />

C. Criteria for Assessing and Reporting Possible <strong>Pandemic</strong> <strong>Influenza</strong> Cases<br />

NYSDOH will develop and distribute to healthcare providers the current CDC and<br />

NYSDOH recommendations for enhanced surveillance, case reporting, and laboratory<br />

testing. The criteria listed below are based on the current information known about the<br />

novel H5N1 influenza virus. These criteria may need to be modified throughout the<br />

pandemic phases according to the circulating virus and the known epidemiology of the<br />

infection at that time. It is anticipated that individual case reporting will not be feasible<br />

once pandemic influenza has been confirmed in New York State. Surveillance during the<br />

pandemic period will focus on data collection mechanisms to assess morbidity and<br />

mortality. Select individual case investigations may need to be conducted to guide<br />

prevention and control recommendations.<br />

Medical provider activities:<br />

• Providers should question all patients who present to health care settings with fever<br />

and respiratory symptoms regarding possible travel and occupational exposure to<br />

influenza A (H5N1) or other novel influenza virus (see Section 5: Clinical<br />

Guidelines).<br />

Criteria for Assessing a Possible Novel <strong>Influenza</strong> Case:<br />

o Clinical Criteria:<br />

• Severe illness: Hospitalized with severe illness, including pneumonia or<br />

ARDS<br />

• Mild illness:<br />

♦<br />

♦<br />

Fever (temperature > 38° C or 100.4° F) and<br />

One or more of the following: sore throat, cough, or dyspnea<br />

o Epidemiologic Criteria:<br />

• Travel Risk (within 10 days of symptom onset)<br />

♦ Visited or lived in an area affected by highly pathogenic avian influenza<br />

A outbreaks in domestic poultry or where a human case of novel<br />

influenza has been confirmed.<br />
