o A description <strong>of</strong> any stranded fish observed. If there are salmonids stranded, the<br />

applicant will contact NMFS PRD immediately to determine if a fish rescue action is<br />

necessary. CDFW will also be contacted with fish rescue information and/or<br />

mortalities by species.<br />

3. Annual Report<br />

Annually, <strong>Reclamation</strong> or its designee will prepare a report summarizing results <strong>of</strong> projects<br />

implemented under the Program during the most recent construction season and results <strong>of</strong> postconstruction<br />

implementation and effectiveness monitoring for that year and previous years. The<br />

annual report shall include a summary <strong>of</strong> the specific type and location <strong>of</strong> each project. The<br />

report shall include the following project-specific information:<br />

A summary detailing fish relocation activities, including the number and species <strong>of</strong> fish<br />

relocated and the number and species injured or killed.<br />

A map indicating the location <strong>of</strong> each project<br />

The number and type <strong>of</strong> instream structures implemented within the stream channel.<br />

The size (acres, length, and depth) <strong>of</strong> <strong>of</strong>f channel habitat features enhanced or created.<br />

The length <strong>of</strong> streambank (feet) stabilized or planted with riparian species.<br />

The number <strong>of</strong> culverts replaced or repaired, including the number <strong>of</strong> miles <strong>of</strong> restored<br />

access to unoccupied salmonid habitat.<br />

The size on number <strong>of</strong> dams removed, including the number <strong>of</strong> miles <strong>of</strong> restored access<br />

to unoccupied salmonid habitat.<br />

The distance (feet) <strong>of</strong> aquatic habitat disturbed at each project site.<br />
