As shown <strong>in</strong> Figure 5.2, 5 major culture issues were raised bythe 20 sample companies. <strong>The</strong>se issues ranged <strong>in</strong> frequency from 65 percent (13 companies raised theissue) to 35 percent (7 companies raised the issue); lack of accountability was the culture issue citedmost often. A brief statement of these five issues follows:1. Lack accountability (65 percent). "We lack accountability, credibility, and consistency at all levels;there's no sense of urgency <strong>in</strong> meet<strong>in</strong>g commitments." Note that this issue is similar to management andoperations issue 1, lack of management accountability and authority, cited <strong>in</strong> Chapter 4.2. Don't empower staff (60 percent). "We don't empower our staff, give them roles, responsibility, andauthority and allow them the opportunity to make mistakes; we don't let managers manage."3. CEO doesn't delegate (55 percent).
