Completion Instructions <strong>for</strong> Form 6 RegisterPage No. A running page no. will be given at the top corner of each page of the register.Col. 1 – Application No.: This will be a running serial number, starting from 1, <strong>for</strong> the applicationsreceived in Form 6. Each application received after final publication upto the next draftpublication shall be continuously numbered.Col.2 – Date of presentation of the Application. The date should be after date of finalpublication.Col.3 –Col.4 -Col.5 -Col.6 –Col.7 –Col.13 –Col.14 –Col.15 –Col.16 –Col.17 -Co.18 -In areas where there is no house numbering, House No., if any, available in therelevant part of the electoral roll in respect of any other family members of theapplicant should be filled up by the official receiving the application. This is necessarywhenever the supplements are integrated to bring the person under the samehousehold.Indicate the letter “F/M/H/O”, as the case may be, in bracket after the name of therelation.Indicate complete EPIC no. with full prefix.The electoral roll part that covers the address of the applicant should be mentioned bythe official from the current electoral roll.<strong>Electoral</strong> roll part is further sub-divided into sections <strong>for</strong> every separately identifiablelocality within the Part and each such section is having distinct numeric number. Theofficial should indicate the no. & name of the relevant section after checking from thecurrent electoral roll.The serial number of accepted claims should be one more than the previous existingserial number in the relevant part of electoral roll. Subsequent applications <strong>for</strong> samepart to be consecutively numbered.Intimation of decision taken should be given in the prescribed <strong>for</strong>m within 3 days.The applications received in a calendar month should be disposed of during the nextmonth and the manuscript should be prepared <strong>for</strong> every part separately, in duplicate,by hand by 25 th of that month. The <strong>for</strong>mat of the manuscript will be same as that ofthe electoral roll.One copy of the manuscript of additions should be attached below the relevant part andthe other copy should be sent to the center, where electoral roll database is maintained<strong>for</strong> data entry.The data entered will be stored in the database of the relevant Assembly constituency<strong>for</strong> printing at the time of next draft publication or on the last date <strong>for</strong> filingnominations in case there is a bye-election from the constituency.Normally during continuous updation, electors who have shifted their residence eitherwithin the constituency or have shifted in from other constituency will file applications.Each such application will have corresponding deletion of the entry in the relevantpart of electoral roll of the constituency of previous residence. Action should be takenafter disposal of the claim application <strong>for</strong> intimating the ERO concerned <strong>for</strong> deletionof the other entry. This should be done in a time bound manner and as such all suchintimations should be given within one month thereafter.208
